Our care teams travel directly to patients who reside in Assisted Living, Memory Care and Group Home communities throughout Minnesota, Wisconsin and Florida and are supported by clinical operations and administrative colleagues who work remotely or at our corporate offices in Stillwater, Minnesota, and Tampa, Florida.

Position Overview:

The Vice President of Care Coordination leads the Care Coordination Business at Bluestone Physician Services. This includes customer relationships, ensuring compliance with all delegated functions based on health plan contracts, accountability for quality of care and financial goals of the department. This position collaborates across Bluestone to ensure operational efficiencies, member-centered care, and sustainability. This position will be part of the senior leadership team and will collaborate on the success of the organization as a whole while focusing on Care Coordination.

Location: Although primarily a remote position, preferred candidates will reside in the Twin Cities metropolitan area with the ability to attend Bluestone's corporate office in Stillwater, MN on a weekly basis

Salary: $150,000+, total executive compensation plan will be commensurate with experience.

Responsibilities:
  • Leads the Care Coordination business and its senior management team
  • Ensures that staffing and financial goals are met with full P&L accountability
  • Responsible to develop and execute on growth plans
  • Ensures efficient and appropriate collaboration of the Care Coordination department as a whole
  • Ensures policies and procedures are updated and align with recent changes as the industry evolves
  • Provides oversight of training objectives and accomplishments that results in staff competency
  • Demonstrates working knowledge of all facets of role, including organizational and departmental policies and procedures and relevant regulations
  • Manages Health Plan contracts for the Bluestone Care Coordination department
  • Ensures care coordination services meets all regulatory requirements set by the payor and CMS
  • Develops processes and procedures for integrating Care Coordination, Primary Care, and external organizations
  • Ability to effectively lead projects from inception through implementation and assessment

Qualifications:

Education/Certification/Experience

  • Bachelor's degree in nursing, social work, or health care administration, Master's degree in business administration or healthcare administration is highly preferred
  • 7+ years of progressive experience in management roles
  • Experience in a health care or social services organization with P&L ownership
