- Oversee and manage the care provided to patients in various settings, including acute care, home health, and home care.
- Ensure that all care provided meets the highest standards of quality and is in compliance with relevant regulations and guidelines.
- Develop and implement care plans for patients based on their individual needs and goals.
- Conduct regular documentation reviews to ensure accuracy and completeness of medical records.
- Collaborate with other healthcare professionals, such as physicians, nurses, and case managers, to coordinate patient care and ensure seamless transitions between different levels of care.
- Stay up-to-date with advancements in medical knowledge and technology to provide the best possible care to patients.
Skills:
- In-depth knowledge of anatomy and physiology to understand the complexities of the human body and its systems.
- Strong managerial skills to effectively lead a team of healthcare professionals and ensure efficient operations.
- Experience in acute care settings to handle complex medical cases and emergencies.
- Familiarity with home health and home care services to provide appropriate care in a patient's home environment.
- Proficient in documentation review to ensure accuracy and compliance with regulatory requirements.
- Excellent communication skills to effectively collaborate with interdisciplinary teams and communicate with patients and their families.
- Strong case management skills to coordinate care across different healthcare settings.
Requirements:
- Bachelor's degree in a related field (e.g., nursing, healthcare administration) required; Master's degree preferred.
- Valid licensure/certification as required by state regulations (e.g., Registered Nurse, Licensed Practical Nurse).
- Strong leadership abilities with a focus on providing compassionate and patient-centered care.
