POSITION SUMMARY:
Responsible for providing quality professional care management services to all clients and their responsible parties adhering to the Aging Life Care Pledge of Ethics, Standard of Practice and Palm Beach Care Management's mission statement. This includes care coordination, managing home health aides, medical oversight, interfacing with medical personnel, advocacy, information, and referrals. Integrate with other resources within and outside of Palm Beach Care Management.
Our objective is to assist our clients in managing and navigating challenges in aging in place as well as assisting adults with physical and mental disabilities and providing the highest quality of life.
QUALIFICATIONS:
RN or Master's Degree in Social Work *Experience in local human service and medical field.
Excellent communication, organizational and time management skills. *Personal integrity and ethical standards.
Must have reliable transportation * Computer proficiency in Microsoft Word, Excel, and Outlook.
Excellent written and verbal communication skills. * Knowledge of physical and mental aging process.
Professional and positive approach, commitment to customer service. *Self-motivated and work with own initiative.
Strong in building relationships, team player and able to communicate at all levels. *Dynamic and Creative.
Recognizes industry trends and problem solving. *Manages time effectively.
*Respectful of company and client confidentiality; any violation of company or client confidence is immediate grounds for dismissal.
Personalized and compassionate service - focusing on the individual client's wants and needs.
Ability to provide non-directive guidance and facilitate constructive relationships.
Demonstrates flexibility and accessibility - working hours are Monday through Friday 830am-5pm, however a company representative is available 24 hours a day, 7 days a week to assist clients.
Ability to ensure inappropriate placements, duplication of services, and unnecessary hospitalizations are avoided.
Quality control - care management services adhere to Aging Life Care Associates Standards of Practice and Pledge of Ethics.
Ability to provide coordinated communication between family members, doctors and other professionals, and service providers.

Compensation: $35.00 - $50.00 per hour
