This position performs outpatient pharmacy functions for the full range of pharmacy services provided by Fort Defiance Indian Hospital Board, Inc. to include controlled investigational, highly potent, high alert, look-alike, sound-alike, chemotherapeutic and/or other medications requiring special handling. Duties:

Reviews medications ordered by clinic providers, patient medication profiles, and patient Electronic Health Records to determine appropriateness, safety and completeness of medication order, authenticity of prescriber, possible drug interactions, potential for adverse reaction, and any other special aspects or considerations in dispensing medications per FDIHB Pharmacy outpatient medication dispensing policy. Consults with providers regarding discrepancies found within/between Computerized Physician Order Entry System (CPOE), Electronic Health Record (EHR) and RPMS. Provides drug information to providers, nursing, family and other healthcare providers utilizing hardcopy, online and other drug reference materials as necessary and appropriate. Participates with physicians in determining course and preferred disease state management. Reviews appropriateness of medication therapy regimen. Advise providers of alternative or preferred medications for cost-effectiveness and/or disease state management standards. Reviews the use of non-formulary and/or restricted items for appropriateness and/or approval by the Pharmacy and Therapeutics committee. Compounds or oversees compounding of medications as prescribed either individually or in bulk. Determines validity of formulations, dosage forms and quantities of dosage forms to be used to prepare desired concentration and quantity and will assign appropriate expiration dating. In addition, will research at the request of providers, compounding recipes for special compound orders. Conducts necessary patient education concerning the medication, its administration, signs of reaction, and additional relevant materials per applicable standards concerning patient counseling (i.e. Show-and-Tell method). Counsel patients on course of treatment and indication for treatment as necessary. When prompted by patients, will provide disease state education to further educate patients on management and treatment options. Inspects processed medications for integrity of dosage form; correct labeling, handling and storage instructions; and proper medication and quantity as labeled of each prescription before dispensing. Reports and document medication errors and/or interventions per FDIHB Pharmacy policy. Inspects bulk pre-packed medications before storage and performs monthly inspections of assigned drug storage areas. Performs other duties as assigned.

Qualifications:

EXPERIENCE: No additional experience required outside of a completed training program as defined in the education qualifications listed below. EDUCATION: Completion of an accredited college or university with a Bachelor's degree in Pharmacy or Pharm.D. Active, unrestricted Pharmacy License in any U.S. State or Territory by completing necessary Board of Pharmacy requirements for the respective state of licensing.
