SUMMARY:
The Physical Therapist is responsible for identifying patient care needs and priorities for care through evaluation, planning, providing and/or delegating care for patients with physical and/or cognitive disabilities to improve functional independence. This process focuses on patient/family needs and assessing the patient's response to care and progress toward outcomes.
RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:

*Participates in meetings, committees and lean projects as assigned.
*Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
*Performs service excellence initiatives such as: AIDET, Key Words, Managing up, 10/5 Rule and Service Recovery when applicable.
*Performs and documents an initial patient examination and evaluates the data to identify problems.
*Establishes, documents and provides a plan of care with functional goals based on evaluation of the examination data and patient needs.
*Provides or delegates and supervises physical therapy patient intervention in a manner consistent with the examination data, the evaluation, and the plan of care.
*Provides and documents patient/family education regarding current condition and plan of care, including home program/discharge instructions.
*Performs and documents patient reexamination, reevaluates the data, and modifies and documents the plan of care.
*Communicates relevant patient care information with other health care professionals.
*Maintains current clinical competency per department guidelines.
*Assists with development and presentation of department community education programs.
*Uses Performance Improvement principles to help improve department operations.
QUALIFICATIONS:
Graduation from an APTA accredited school of Physical Therapy required. Current Physical Therapy license with the State of Nebraska required. Current Basic Life Support (BLS) Certification required.
