CLASSIFICATION: Salaried Exempt TRAVEL / LOCATION: Buffalo, NY

JOB SUMMARY: The Nurse Practitioner is responsible for providing care for patients (pediatrics, prenatal, adult) in collaboration with JRCHC physicians consisting of assessment, interpretation of data, and development and implementation of a therapeutic plan.

QUALIFICATIONS:
  • RN, Masters or Nurse Doctorate (ND) Certification by American Nurses Credentialing Counsel (ANCC), American Academy of Nurse Practitioners, National Board of Pediatric Nurse Practitioners or equivalent certifying body.
  • 2-3 years of experience preferred
  • Ability to work under conditions of OSHA Category 1 - involving exposure to blood, body fluids, or tissue
  • Responsible for delegating tasks and responsibilities to appropriate persons.
  • Read current literature, talk with colleagues, or participate in professional organizations or conferences to keep abreast of development in nursing.
  • Maintain current knowledge of state legal regulations for nurse practitioner practice, including reimbursement of services.
  • Maintain departmental policies and procedures in areas such as safety and infection control.
  • Advocate for accessible health care that minimizes environmental health risks.
  • Provide patient care in a holistic manner including emotional, mental, and spiritual health.
  • Ability to assess, acknowledge and develop the spiritual health of staff and patients.
  • Ability to adhere to professional code of ethics, and compliance to Article 28 and FWHC requirements.


Essential Job Duties

CLINICAL

ACUTE
  • Perform primary care procedures such as suturing, splinting, recommend immunizations, taking cultures, and debriding wounds.
  • Treat or refer to patients for primary care conditions, such as headaches, hypertension, urinary tract infections, upper respiratory infections, and dermatological conditions.
  • Diagnoses or treat acute health care problems, such as illnesses, infections, or injuries.
  • Recommend diagnostic or therapeutic interventions with attention to safety, cost, invasiveness, simplicity, acceptability, adherence, and efficacy.
  • Detect and respond to adverse drug reactions, with special attention to vulnerable populations such as infants, children, pregnant and lactating women, or older adults.

PREVENTATIVE (WELLNESS)
  • Maintain complete and detailed records of patients' health care plans and prognoses.
  • Analyze and interpret patients' histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses.
  • Perform routine of annual physical examinations.
  • Prescribe medications based on efficacy, safety, and cost as legally authorized, and medication dosages, routes, and frequencies, based on such patient characteristics as age and gender.
  • Counsel patients about drug regiments and possible side effects of interactions with other substances, such as food supplements, over-the-counter (OTC) medications, or herbal remedies.
  • Diagnose or treat chronic health care problems, such as high blood pressure and diabetes.
  • Develop treatment plans, based on scientific rationale, standards of care, and professional practice guidelines.
  • Taking and recording vital signs and medical histories, or information such as test results in medical records.
  • Preparing patients for examinations, drawing blood, and administering medications as directed by providers.
  • Collect samples (such as blood, urine, or sputum) from patients and perform routine laboratory tests.
  • Complete all patient charting and ensure all patient records are current/complete with strong attention to detail.

ADMINISTER TESTS & EVALUATE RESULTS
  • Follow up with patients regarding lab results or other medical care concerns (i.e. Pap Smear follow up on women).
  • Order, perform, or interpret the results of diagnostic tests, such as complete blood counts (CBCs), electrocardiograms (EKGs), and radiographs (x-rays).

PATIENT COMMUNICATION & EDUCATION
  • Be aware of existing agencies and resources in the community and will refer patients to these resources as indicated.
  • Educate patients about self-management of acute or chronic illnesses, tailoring instructions to patients' individual circumstances.
  • Provide patients with information needed to promote health, reduce risk factors, or prevent disease or disability.
  • Recommend interventions to modify behavior associated with health risks.
  • Consult with, or refer patients to, appropriate specialists when conditions exceed the scope of practice or expertise.
  • Provide patients or caregivers with assistance in locating health care resources.
  • Assist and participate in the work of community health projects and educational programs.
  • Be aware of existing agencies and resources in the community and refer patients to these resources as needed.

COORDINATION / POST VISIT
  • Meet regularly with the physician to coordinate health care plan and review and evaluate the collaborative efforts.
  • Provide care for patients (pediatrics, prenatal, adult) in collaboration with JRCHC physicians consisting of assessment, interpretation of data, and development and implementation of a therapeutic plan.

TEAM
  • Attend weekly staff meetings a monthly provider meeting except when clinical activities prevent such attendance.
  • Assist and participate in the work of community health projects and educational programs.
  • Supervise of coordinate patient care or support staff activities.


PHYSICAL REQUIREMENTS: Must be able to sit or stand for long periods of time. Exert up to 50lbs. pushing, pulling, or lifting. Walking, climbing stairs required. Able to talk/hear communicate with others with or without reasonable accommodation.

TRAVEL REQUIREMENTS: Must be able to travel to and between sites while maintaining standards of safety and compliance with motor vehicle traffic laws. This position may require travel for necessary training and conferences.
