What You Will Do
As the SNF-ist, you will act as a Skilled Nursing Facility's leading provider of internal medicine services. You will collaborate with the facility's interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all of the facility's residents.
You will work with the entire facility's patient population - short and long-term residents - overseeing their day-to-day care with the assistance of the facility's nursing staff and guidance from the facility's medical directors and attending physicians.

As a SNF-ist, you will provide types of care, including but not limited to:
- Be comfortable and proficient with documentation and CPT coding
- Evaluation of new-admission visits
- Coordination of discharge visits
- Monthly stability visits for long term residents
- Evaluations for patients exhibiting changes in condition
- Advanced Care Planning
Other Duties & Responsibilities
- Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease
- Prescribe all necessary medications and treatments
- Perform comprehensive physical examinations of patients
- Develop and implement patient management policies and procedures
- Facilitate referrals to other healthcare professionals and medical facilities
- Communicate with attending physicians and/or specialists regarding patient care
- Be comfortable and proficient with documentation and CPT coding

Our Requirements
Licensure / Certification / Registration:
- Advanced Practice Licensure (APN) with an applicable state license
- Must have an active license and DEA #
- Previous experience in nursing or other medical fields
- Minimum one-year experience in a Skilled Nursing setting (preferred)
- Ability to build rapport with patients and their loved ones
- Ability to thrive in a fast-paced environment
- Excellent written and verbal communication skills
- Strong leadership experience with emphasis on teamwork in a healthcare setting
Education
- Master's degree required
