Salmon Hospice is seeking a hard-working, organized, and dedicated Registered Nurse to join our cohesive, growing team as Hospice Clinical Team Lead. The ideal candidate will have hospice experience, possess excellent communication skills, and demonstrate the ability to encourage, motivate, and educate staff to reach their full potential and consistently deliver high quality patient care. In addition, they will be adaptable, flexible, have a working knowledge of Hospice regulations and requirements, as well as possess the curiosity and determination to grow as a professional.
Working under the Hospice Director, this position involves collaborating with the current Hospice Clinical Team Lead to each oversee the care provision of a dedicated group of clinicians within a geographical area.
The position is Monday-Friday 8am-4pm, in-person at the office in Milford, MA, with some weekend/holiday hours required per on call rotation (remote).

Qualifications
- Licensure: Registered Nurse Licensed to practice in Massachusetts
- Experience: Minimum 3 years clinical experience
- Minimum 1 year experience in a leadership role
- Hospice experience required
Duties to include:
- Leading IDT for dedicated team (opposite week of other Hospice Clinical Team Lead)
- Tracking Face to Faces for dedicated team and ensuring their timely completion
- Participate in orienting and mentoring new staff
- Completing workflow, reviewing documentation, and ensuring all are completed properly per Medicare Regulations
- Participate in staff education and training including QAPI/PI
- Screening referrals and assisting in setting up admissions
- Answering clinical calls from families, staff, and outside entities in a compassionate manner
- Tracking and compiling data to ensure quality care and compliance with regulations
- Ensuring field staff visit schedules are adequate to meet patient clinical need and productivity goals, assigning visits to LPN's on a daily basis
- Participating in manager on call weekend/holiday rotation
- Other duties as assigned
