Bellin Health is seeking a Family Medicine Physician to join a long standing practice with a well established patient base and excellent support staff at our Denmark and Wrightstown Clinics. This split role is a wonderful opportunity to practice medicine in smaller communities while having the support of a large health system. A Team Based Care model and a 4 day work week make this an opportunity that can't be passed up.

Shift Hours:
1.00 FTE (40 hours/week), Days, rotate between Denmark and Wrightstown
Practice Details:
- Full-time practice: 40 hours per week with a minimum of 32 hours available for patient appointments, minimum 4 days per week;
- 18-22 patients per day
- Epic access - remote access from home
- 2-year compensation guarantee, plus production, quality and value-based bonuses
- Time off: 160 hours per fiscal year; 264 hours per fiscal year once compensation guarantee expires
- CME stipend of $5,000 per fiscal year and 80 hours of time off
- Participate in a large call group, phone call only
- No weekend or holidays, except for on-call weekends
- Teaching opportunities available with medical students from the Medical College of Wisconsin-Green Bay
- Up to $12,000 for relocation assistance
