Main Function
Under the direction of the Pre/Post Manager the Total Joint Navigator is responsible and accountable for planning, organizing, and monitoring operations of the Total Joint Replacement Program's Day to day responsibilities to achieve and improve patient through-put, outcomes, provisions of care and services. As the clinical experience liaison for the patient, caregivers, families, and physician offices this position is responsible for planning, coordination, integration, education, and the ongoing evaluation of delivery of care to the specialty orthopedic population in need of total joint replacement across the ASC service area. The Navigator is responsible for assuring delivery of integrated, orthopedic care from the initial contact with the patient, pre-operative, peri-operative and post-operative settings. The total Joint Navigator drives care coordination by actively providing consultative screening, evaluation, and education of appropriate patients to direct and enhance access to ASC services and improve patient, family, and physician satisfaction.

10. Duties and Responsibilities
Responsible for the overall administration and operations of the Total Joint Program under the direction of the Pre / Post Manager Ensures the timeline and key deliverables of the implementation plan are achieved Works in collaboration with physicians and physician office personnel for marketing and outreach to conduct community education to increase awareness and grow market share. Conducts pre-surgery education classes for patients and caregivers Conducts face-to-face evaluation of patient and ensures patient meets admission criteria for ASC Works closely with Anesthesia to ensure face-to-face evaluation takes place. Provides discharge instructions to the total joint patient and family on day of surgery Coordinates all aspects of home care related to Home Health, DME and PT and ensures all meet DOS criteria Works with Managers to ensure patient satisfaction and quality of care throughout patent experience Accountable to maintain education materials to educate patient, caregivers, physicians, office staff and ASC staff to ensure optimal care of patient Works closely with surgeons to ensure protocols are followed and educational materials are current Works in conjunction with BHC Total Joint Program to ensure standardization as much as possible Works with Clinical Managers to develop, define and evaluate competencies to ensure staff is knowledgeable and able to care for total joint patients throughout the patient experience. Maintains a flexible schedule as necessary to meet patient needs Participates in development, implementation and evaluation of Total Joint Program mission and goals Serves as liaison and subject matter resource to physicians, staff, patients, families and professional organizations to ensure optimal patient care is provided Works closely with QA Coordinator to develop data collection methods and processes for tracking and trending quality outcomes Works as a mentor for professional practice and clinical team development related to total joint procedures Demonstrates age appropriate care across lifespan with emphasis on adult needs Participates in appropriate clinical meetings and committees to ensure continuum of care Communicates professionally with patients, caregivers, surgeons, physicians, vendors and staff Adheres to the Credo and Policies and Procedures of the ASC. Performs other duties as assigned Check and respond to emails in a timely and efficient manner Works closely with the Pre/Post Manager, Director of Nursing and Administrator to promote the facility
Qualifications
Bachelor's degree in related field required Graduate of accredited school of nursing Current Florida Registered Nurse License Current BLS, ACLS Advanced Certification preferred (required within 18 months of hire) Ability to generate reports and patient materials related to total joint program
Requirements
Strong knowledge of computers and MS Office programs Previous experience in start-up of program (Dialysis, Home Health, etc.) a plus Ability to generate reports and patient materials related to total joint program Strong ethical and moral character references Excellent Leadership and communication skills
