DIRECTOR of CLINICAL SERVICES / DIRECTOR OF NURSING

Job Description

Plans, directs and participates in delivery of professional services to home care patients.

Provides leadership to clinical staff.

Directs the admission criteria, patient eligibility with various payers and the appropriateness of recertifications and discharges.

Participates in the hiring, orientation and retention of qualified personnel to maintain adequate staffing levels.

Evaluates staffing and supervision needs to ensure staffing pattern is adequate for caseload during all hours of operation.

Evaluates the home care staff job performance.

Provides staff development and inservice/education programs to ensure competencies of the clinical staff.

Assists in the planning and development of agency structure, policies and procedures, and the annual operating budget.

Reviews and updates patient care policies and procedures annually.

Identifies data for performance measurement and improvement activities to evaluate the effectiveness of the program and to ensure quality patient care.

Utilizes OBQI and PI information to improve patient outcomes in a cost effective manner.

Implements systems to assure compliance with applicable laws and regulations and CHAP standards.

Provides input to strategic planning.

Acts on behalf of Administrator in her absence. Appoints person to act in own absence.

Seeks assistance appropriately.

Participates in interdisciplinary evaluation of the patient care and the development and/or changes to the plan of care.

Participates in QAPI process.

Fulfills additional responsibilities when assigned.

POSITION QUALIFICATIONS:

Ability to lead interdisciplinary home care team. Current license to practice as RN in Indiana. Baccalaureate degree in nursing or related field preferred. Minimum of two years clinical experience. Prefer prior home care clinical (1 year) and prior supervisory/management experience. Strong verbal and written communication skills. Ability to lead change.

POSITION DESCRIPTION:

Provides direction, coordination, supervision, and implementation of clinical services to home care patients in a safe, cost-effective manner.

PHYSICAL REQUIREMENTS:

Ability to: bend, twist, extensive sitting; push/pull 150 lbs.; lift/carry 50 lbs.; Visual and hearing sufficient to communicate effectively with staff and customers. Ability to deal effectively with stress and time management.
