Position Summary:
The incumbent is expected to provide expert level psychosocial interventions and case management services to vulnerable older adults with complex medical, social, financial, community and behavioral problems. By engaging and educating families and caregivers involved in the older adult's care, the social worker works to optimize the social supports available to the individual to promote maximum functioning and quality of life. In collaboration with the primary care physician (PCP), the ACO team works closely together to assess the individual, develop and implement a care plan, evaluate and treat geriatric conditions, and provide longitudinal care management and coordination of care. As an independent practitioner of social work, the social worker evaluates psychosocial needs and applies social work interventions with a high degree of autonomy, exercising independent judgment and initiative in carrying out social work functions and in applying social work values and ethics to complex situations.

Salary Range:
Annual Rate Minimum $81,828.72 Midpoint $94,899.60 Maximum $107,970.48
Required Qualifications:
Must possess valid California drivers license at time of hire
Ability to establish and maintain effective working relationships across the Health System
Ability to maintain a work pace appropriate to the workload
Must demonstrate customer service skills appropriate to the job
Skills to interpret and implement hospital, JCAHO, Health Department and Child Abuse Registry standards and regulations
Excellent written and verbal communication skills in English
Master's Degree in Social Work
Knowledge of and ability to work within a complex health care system
Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
Preferred Qualifications:
Previous experience working with geriatric patients
Knowledge of University and medical center organizations, policies, procedures and forms
