The salary for this position is $74k-$105k annually and is commensurate with education and applicable experience and certifications. To be a Nursing Home Liaison/Case Manager - RN you need to be:
- Manage a caseload of patients residing in various local nursing homes.
- Complete admissions for hospice-eligible patients residing in nursing homes
- Liaison with nursing leadership at nursing homes to ensure plans of care and medication regimens are communicated and followed.
- Liaison with nursing home leadership to ensure and improve hospice-eligible patient referrals.
- Dedicated to the philosophy of hospice care.
- Believe that excellent patient care is the only acceptable standard.
- A Registered RN, licensed to practice nursing in New York State.
- Comfortable providing community-based care.
- Some experience caring for terminally ill patients and a basic understanding and commitment to the hospice philosophy of care.
- One to two years of recent bedside nursing experience with good clinical skills.
- Ability to assess the needs of terminally ill patients and their families and to plan care to meet these needs.
- Ability to relate to a diverse population and workforce in a personable and effective manner.
- Love to work on a team of dedicated professionals from multiple disciplines.
- Must possess a car, valid driver's license, and current insurance and arrange other means of transportation if your personal car is unavailable.
