Senior Care Pharmacy Services has been doing business in Central Illinois since 2004, Servicing over 2100 residents in Skilled Nursing and Assisted Living Communities. Service location include 4 states, Illinois, Iowa, Missouri and Indiana. Senior Care provides same day delivery and overnight shipping to our Iowa and Missouri communities. The pharmacy is open 7 days a week and provides after-hours service as needed. Senior Care has approximately 35 employees.
Purpose:
Interact with professional, technical and lay staff and clients in an effort to promote and maintain superior standards of quality that is professionally acceptable to the pharmacy and its consumers.

Responsibilities
Essential Tasks:
- Perform Drug Regimen Reviews monthly or on all customers in all centers served as dictated by federal and state laws and regulations.
- Provide nursing inspections for all units and medication storage areas, including medication/treatment carts and emergency drug supplies as dictated by federal and state laws and regulations.
- Provide in-service education programs to medical and nursing staff in order to keep staff abreast of the latest trends in pharmaceutical care and to address the needs of the elderly, specific disease management and the latest in regulatory and compliance issues.
- Provide Drug Utilization Reviews to examine the cost effectiveness of a medication or group of medications to improve outcomes and decrease costs.
- Provide guidance with medication pass observations of nurses and/or certified medication technicians administering medication to customers as requested.
- Monitor the application of pharmacy policies and procedures as they relate to communities.
- Assist communities with adverse drug reporting, controlled substance destruction and responding to pharmaceutical services caused survey deficiencies.
- Attend meetings related to Pharmacy Services, Quality Assurance, Infection Control and specific customer care.
- Organize and maintain all records necessary.
- Complete "Pharmacist Monthly Consultation Plan" including signature and time of appropriate community personnel upon entering and exiting the community for scheduled consultation visit.
- Maintain and service company vehicle; including routine maintenance and unexpected repairs.
- Come to work as scheduled and consistently demonstrate dependability and punctuality.
- Participate in in-services and activities as required.
- Observe all policies and procedures, including but not limited to safety, infection control and those contained in the employee handbook.
- Maintain the confidentiality of all protected health information whether electronic, written or oral exposed to either during the course of assigned duties or as a result of an incidental disclosure.
- Apply the Minimum Necessary Standard in all matters related to Residents' protected health information.
Qualifications
The Successful Candidate Will Have
- BS in Pharmacy required.
- Able to travel to all communities serviced by SCPS as required.
- Current license to practice pharmacy required, Will be required to completed licensure in Illinois, Iowa, Indiana, Missouri (company will re-imburse all cost)
- Two (2) years' experience in providing services to long-term care centers OR at least two (2) years' Clinical Pharmacy experience at a hospital or community pharmacy required.
- Proficient in clinical interpretation, drug-drug, drug-disease, drug-food interactions and allergies.
- Knowledge of the American Society of Consultant Pharmacist guidelines for practice, state and federal regulations, and drug indicators for long-term care centers required.
- Knowledge of the physiology of the aging process and how it affects drug therapy required.
- Previous Consultant Pharmacist experience preferred.
- Able to follow written and oral directions.
- Ability to work independently.
- Possess the ability to communicate effectively and deal tactfully with all customers and co-workers.
- Pass background check before and during employment, as defined by Senior Care Pharmacy's background check policy, with no disqualifications by committing or attempting to commit one or more of the offenses defined within.
