Position Duties and Responsibilities:
- The Government reserves the right to assign the Clinical Pharmacists supporting this task order to provide services at the Pain Management and Primary Care, or Medical Home Port Teams at Naval Hospital Jacksonville (NHJ), FL, to include the Satellite Pharmacy and Branch Health Clinic located on Naval Air Station Jacksonville, FL.
- The HCW shall provide services within the scope of the clinical privileges granted by the commanding officer, the applicable duties provided in the basic contract.
- Work closely with clinical staff in developing patient wellness programs such as pain management, behavioral health, sleep hygiene, and smoking cessation programs.
- Evaluate drug literature to compare and contrast therapeutic regiments and uses for new and existing products.
- Utilize pharmacy electronic health record (EHR) computer and robotic systems for dispensing medications, document patient care and review patient care rendered by medical providers in other clinics or MTFs in accordance with MTF protocols.
- Additional duties include but are not limited to: developing understanding of the pain stepped-care model. Tricare Uniform Formulary Tool, formulary and non-formulary medication screening, pain clinical interventions, appropriateness of therapy, general understanding of workflow operations, participate in customer service initiative, as non-voting member, in Patient Safety Month, medication adverse event reporting and tracking, technician education and guidance, patient counseling, educational presentations to staff, medication reconciliation, and clinical pharmacy if supplemental privileges have been granted.
- The HCWs shall be responsible for the quality and timeliness of records, encounter closures within 72-hours, reports, and documentation of services provided. Document and communicate all information as appropriate including pain patient agreements, pharmacotherapeutic treatment findings and recommendations.
- Assess patient's response to drug therapy and planning drug therapy based on physician established diagnoses. Consult with other healthcare providers (e.g., physicians, dietitians, nurses, physical therapists, etc.) about patient treatment needs or options. Conduct a deliberate medication review and reconciliation and develop strategies for patients with histories of non-compliance, abuse and/or diversion.
- Have knowledge of pharmacokinetic and pharmacogenomic dosing as it pertains to complex medication regimens including but not limited to, opioids, antidepressants, antipsychotics, and substance use disorder medications.
- Make evidence-based clinical recommendations to providers using a courteous, proactive approach to effective patient centered care. Participate, as a non-voting member, in a multidisciplinary team with other health care providers providing consultative services pursuant to optimal health care delivery to patients with complex medication regimens including to provide drug therapy recommendations, perform polypharmacy management, and provide patient education regarding changes in drug therapy.
- Provide, assess, and review information regarding the general administration of medications, safety, efficacy, toxicity, interactions, investigational drugs and bioavailability. Monitor and review drug therapy for rational indications, dosing, adverse drug reactions, and complications of multiple drug therapies in all patient care settings.
- Provide pharmacologic and non-pharmacologic therapies to include prescribing in accordance with DHA, DoD, and MTF policy. Auricular acupuncture may be performed as treatment, if the HCW possess the appropriate training and certification per DHA PI 6025.52, and the HCW is granted the appropriate clinical privileges by the MTF Director, as required for comprehensive pain management. Clinical Pharmacist issued supplemental privileges shall develop appropriate treatment plans and monitor results assessing the patients' outcome to the suggested therapy.
- Order consults and referrals as appropriate.
- Participate in tele-mentoring, tele-medicine, and tele-consults.
- Conduct limited physical assessments relevant to the patient's medications and comorbidities. Examples of this may include but are not limited to, vital signs and diabetic foot exams. Perform opioid use screenings and questionnaires such as
- Patient Health Questionnaire-2 (PHQ2), Patient Health Questionnaire-9 (PHQ9), General Anxiety Disorder-7 (GAD7), Opioid Risk Tool (ORT).
- Facilitates quality safety measures for pain management including but not limited to review of controlled substance use agreements, need for urine drug screens, need for opioid-reversal medication, and high-dose opioid use.
- Design and conduct interdisciplinary educational programs in the areas of drug therapy for physicians, nurses and other health professionals.
- Collaborate on preceptorship duties with the Pharmacy Clinical Coordinator for pharmacy residents/students assigned to the Pharmacy Department.
- Provide activity support to Naval Hospital Jacksonville committees and Quality Management Boards including, but not limited to, Long-term Opioid Therapy and Safety (LOTS) Committee, Pharmacy and Therapeutics (P&T) Committee, medication use, nutritional support, The Joint Commission tracers, and utilization review by gathering information and drug use related to quality assurance of patient care. Contractors are not authorized to participate as a voting member of any Board or Committee.
- Support investigational drugs and research programs as needed.
- Adhere to the Customer Service Accountability - Contract employees providing services at NH Jacksonville and supporting facilities shall adhere to the Customer Service standards specified in DHA policies and procedures and NAVHOSPJAXINST 6320.7H or updated versions. A copy of the instruction will be available upon request or at the time of commencement of services.
Minimum Qualifications:
- Doctor of Pharmacy (Pharm.D.).
- Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).
- Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy or documentation of appropriate education, training, and/or CME in the practice of clinical pharmacy.
- Possess a minimum of 12-months of experience within the preceding 36 months as a Clinical Pharmacist performing direct patient care clinical activities in an ambulatory care pharmacy setting.
- Current, full, active, and unrestricted license to practice as a pharmacist.
- Must be a US Citizen.
