POSITION SUMMARY
A Clinic Nurse Practitioner or Physician Assistant is responsible for managing health problems and coordinating health care for patients in accordance with state and federal rules and regulations. This role assesses health status, diagnoses, develops plans of care and treatment, implements treatment plans, and follows up with evaluations of the patient's status. The NP/PA uses clinical judgments in conducting health assessments, making different diagnoses, and prescribing pharmacological and non-pharmacological treatments. Plans are implemented through independent action, health counseling, and collaboration with other healthcare providers.

MINIMUM QUALIFICATIONS
Education:
- Master's Degree or Post-Master's Certificate in a relevant Nurse Practitioner Program Certified by either ANCC or AANP Board, including completion of all requirements for Continuing Medical Education (CME) and re-certification.
OR:
- A Graduate of an Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) approved Physician Assistant training program.
Experience:
- Previous primary care in a clinical setting is strongly preferred.
Licensure/Certification:
- Must possess a current license as a Nurse Practitioner from the West Virginia Board of Registered Nurses and must possess and maintain a current RN license. OR
- Must possess current license as a Physician Assistant by the WV Board of Medicine. Certification by the National Commission for Certification of Physician Assistants (NCCPA).
- Must have or be able to obtain prescriptive authority.
- Must be credentialed according to the Grafton City Hospital Medical Staff By-Laws and appointed as an Allied Health Professional Affiliate Staff member.

- Identify the teaching/learning needs of patients.
Competency Patient Care
- Develop and implement care plans based on findings and examinations.
- Prepare patient summaries, collect specimens, and carry out the analysis of cultures and tests.
- Review medical documents from outside medical providers and facilities.
- Prepare discharge instructions if they have been directly involved in patient care.
Competency Knowledge and Skills
- Know what clinical procedures need done and how to do them.
- Provide counseling and instruction regarding common patient problems, limitations, and abilities.
- Identify normal and abnormal findings in history, physical examination, and commonly performed laboratory studies
Competency: Technology
- Have basic computer skills and demonstrate proficiency in documenting and looking up clinical information in electronic records.
Competency Safety
- Protects patients, visitors, and employees by adhering to all policies and protocols, and identifying patients using two identifiers (i.e., name and date of birth) prior to providing any treatment or procedure. Consults supervising provider as needed.

- Frequents lifting and/or carrying objects weighing up to 50 lbs. with a maximum lifting of 100 lbs.
