As the Director of Post-Acute Services, you will play a critical role in continuing the healing mission of Deaconess.

Job Overview:

Our current post-acute patient journey is difficult to navigate while being critical to our patients' success as well as that of our growing health care system. Patients are often at a loss as who to call or where to go once they leave the hospital. This often results in patient frustration and dissatisfaction, a delay in providing the right level of care, and readmissions. We tend to work in silos, with communication challenges and team members across the system performing duplicate work.

More than one in five Medicare patients discharged from a hospital receives post-acute care. The total impact of post-acute services on readmission, ER visits, outcomes and overall spending, far exceeds the direct cost of post-acute services. Because value based payment models increasingly focus on costs and outcomes occurring inside and outside hospital walls, now is the time to rethink post-acute care strategies to drive value. Market pressures further reinforce the need to take a new look at post-acute care.

This new position will be responsible for Home Health, the Post-Acute referral team, Palliative Care, Hospice, our Preferred Provider network and Transitions of care. Restructuring our post-acute services will minimize the cost impact to the health system on the front end, while increasing our responsiveness, reducing complexity of the patient's journey, and adding a focus and standardization of post-acute processes immediately.

This director will be the champion for all of our post-acute initiatives. This leader should be well versed in the ever-changing governmental programs and requirements surrounding care transitions and be passionate about providing our patients with the right care at the right time at the right place. We have already posted a position for a navigator to help with moving all referral employees to this new service line. By having one referral team and management structure, we are positioned to streamline and standardize processes resulting in faster patient placement with less leakage.

Required Education and Experience:

Minimum of 5 years in a leadership role, preferably focused in post-acute care services. Experience in a complex health system would be preferred.
Master's degree is also preferred in addition to a clinical/social work degree.
Management of a budget and experience in a matrix type of organizational system will be beneficial.
