BASIC FUNCTION:

The Physician coordinates and interacts with the Medical Director, clinical and clerical staff to achieve unification of administrative and clinical functions into an efficient, rewarding, financially stable system of health care according to established policies and procedures through the delivery of direct services, the supervision of other providers, and the coordination of consultative and other referral services. Serves as a provider to patients without discrimination to age or gender. This individual uses decision-making skills to establish a diagnosis and to plan therapy.

SECTION II

EDUCATION AND EXPERIENCE REQUIREMENTS:
  • Membership on the Ellis Hospital Medical staff
  • Appointment to the Department of family Practice, the Department of Internal Medicine and/or the department of Pediatrics
  • Certification by the American Board of Family Practice, the American Board of Internal Medicine, and/or the American Board of Pediatrics is highly desirable, or appropriate governing body based on role.


SECTION III

PHYSICAL REQUIREMENTS:

The position is located indoors under normal working conditions. Should be able to push/pull 25 lb., lift/move 15 lb. from floor to table, be able to perform moderately difficult manual manipulations such as using a key board, writing and filing for extended periods of time, must be able to perform tasks which require hand-eye coordination such as data entry, typing and using photo copiers. Mobility requirements may include the ability to sit at a computer terminal or work station for a prolonged period of time in addition to being able to squat, stand and walk for a reasonable length of time and distance. Sensory requirements include the ability to articulate and comprehend the spoken English language in addition to being able to read the English language.

The Physician provides direct comprehensive primary care serves within the scope of deliniated clinical privileges. This includes assessing, planning, evaluating, and revising care for the patient across the lifetime of the relationship. This may include both ambulatory and hospital care.

Provides treatment, direction and supervision of medical services in accordance with American Medical Association and specialty board standards of practice as well as applicable local, state and federal laws and statutes and in accordance with regulatory and accreditation organizations to assure maximum benefit to patients/ families. Identifies and manages actual or potential problems of patients/families.

2.

Demonstrates knowledge and ability in incorporation of lifespan milestones into plan of care (e.g. age appropriateness, growth and development, aging.)

3.

Demonstrates an appropriate effort to establish a therapeutic relationship with individual patients/families and to facilitate communication leading to improved health status

Collaborates with members of

the healthcare team to facilitate coordinated delivery of care across the continuum. Patients requiring hospital care receive prompt care as medically indicated. Patients/ families requiring after-hours consultation or treatment are promptly contacted.

5.

Refers patients to other levels of specialty care as appropriate when specialized needs are identified.

6.

Utilizes prescriptive authority in a manner consistent with standards of practice.

7.

Demonstrates ability to provide individualized patient/family education related to health maintenance, illness, wellness, and prevention. Presentations are provided to health councils, civic clubs, groups and the schools to educate the community in the services available when requested.

A complete, accurate medical record is maintained following the Problem Oriented Medical Record of all treatment administered to patients within 24 hours of service.

9.

All transcribed patient records are reviewed, corrected, and corrections verified within seventy-two hours of receipt from the transcriptionist to assure that all information is readily available.

Maintain ready availability for consultation with physicians and other health professionals regarding patient assessment, diagnosis, treatment, etc.

Reviews and co-sign records of treatment provided by non-physician providers with in 72 hours of encounter if necessary and appropriate.

Charts, orders, insurance forms, and other paperwork are prepared in a timely and accurate manner and in accordance with written departmental procedures.

The Physician continually improves clinical avenues to provide optimal care while maximizing resources.

1.

Participates in the attainment of budget objectives.

2.

Uses all information available to monitor use of resources, identify trends and develop plans of action to insure efficient practice.

3.

Maintains current knowledge of reimbursement issues as it applies to individual practice.

4.

Seeks avenues for revenue generation in a creative and innovative manner.

5.

Participates on behalf of Ellis HealthCare Centers with third party payers in managed care networks.

6.

All services provided with diagnoses are clearly recorded to assure complete, accurate charges to patients and insurance carriers.

The Physician creates an atmosphere for positive communication across all departments and communities that play a role in the continuum of care.

The Physician will participate in Quality Improvement activities.

1.

Participates in maintaining and revising an on going review process of medical services provided by the healthcare centers to assure quality healthcare to patients in accordance with established policies.

2.

Annually participates in developing methods of improving procedures and eliminating problems based on reviews.

3.

Review, instruct and cooperate in the monthly monitoring system of QI indicators

4.

Participate in periodic evaluation of primary care services.

5.

Participate in designing a provider-staffing schedule to provide medical services in the office and after-hours consultation or treatment, according to policies and procedures of the healthcare center.

E

The Physician will assume responsibility for education and self-development.

1.

Maintains competency in primary care serves within the scope of delineated clinical privileges, and keeps up with new knowledge and advances in the profession.

2.

Attends continuing education programs pertinent to maintenance and development of expertise related to the area of chosen practice and keeps record of own inservice.

3.

Assists in training and education so as to encourage clinical support staff to grow in professional knowledge and capability.

4.

Participates in staff education to implement revised techniques or treatments.

5.

Functions within the standard of practice, policy, procedures and guidelines of Ellis Hospital, including medical protocols and procedures, administrative policies and procedures, and Medical-Staff rules and regulations.

6.

Participates in effort to continuously improve unit operations.

7.

Recognizes job relationship and functions effectively within the organizational structure.

8.

Supports the implementation of yearly educational programs for all professional staff.

Adheres to the policies and procedures as outlined in the Exposure Control Manual regarding:
  • Standard Precautions
  • Bloodborne Pathogen
  • Disposal of Needles, Syringes and Sharps
  • Cleanup Procedures for blood and body fluid spills
  • Storage, Handling and Disposal of Medical Waste

Demonstrates knowledge and understanding of the policies and procedures outlined in the Exposure Control Plan.

2.

Practices safe work techniques and utilizes proper engineering controls to minimize the risk of bloodborne pathogen exposure.

3.

Maintains clean and safe work area free of biohazardous waste.

4.

Appropriate Personal Protective Equipment is utilized when performing a task or responsibility associated with this position which represents a potential occupational risk of exposure to bloodborne pathogens. Appropriate PPE's shall include but not be limited to gloves, gown, goggles, laboratory coat, face shields or masks.

5.

Demonstrates knowledge of the appropriate level of personal protective equipment to be used in relation to the specific tasks and responsibilities.

6.

Degree of compliance as measured by personal observation of immediate supervisor/manager.

7.

Degree of compliance as measured by feedback from other supervisors/ managers.

Adheres to hospital policies and procedures related to:
  • mandatory education
  • annual health assessment
  • licensure verification


1.

Continuing education records will be completed and forwarded to Human Resources annually regarding mandatory educational requirements.

2.

Completes annual health assessment form as required, maintain current inoculations

3.

Current license and registration as required by N.Y. State will be forwarded to Human Resources upon renewal or issuance

Salary Range: $100-120/hour Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
