  • Position Summary
    • The Outcomes Specialist promotes excellence in clinical practice by incorporating clinical homecare expertise to provide oversight to visiting clinicians by ensuring appropriate home health coding, accurate OASIS assessments and complete supporting documentation. The Outcomes Specialist coaches and works collaboratively with clinicians to assess, plan and coordinate care to meet the health needs of patients and improve outcomes. This position reports to the Quality Director.
  • Required Minimum Knowledge, Skills, and Abilities (KSAs)
    • Education: Bachelor's degree in appropriate clinical designation/field required. Three to five years of progressive clinical experience, including one year of home healthcare; quality improvement experience preferred.; Or RN, PT, OT or SLP with more than five years of clinical experience, including more than one year of home care.
    • License/Certifications: Current industry-accepted OASIS certification, or obtain within 6 months of hire.

    • State of Maine licensure in appropriate clinical designation.
    • Current, valid driver's license to operate a motor vehicle, with vehicle available for work covered by liability insurance as specified by agency policy.
    • Current certification in Basic Life Support for Healthcare providers.
    • Demonstrated competency in OASIS and home health coding is essential.
    • Excellent communication and facilitation skills and ability to recognize and manage conflict.
    • Expert knowledge of homecare reimbursement methodology, including Medicare Prospective Payment System (PPS) and proposed pay for performance strategies required.
