RN Case Manager, Emerging Therapies, Outpatient (40 hrs)

Job Posting Description

The Case Manager RN will be responsible for:

• Planning, organizing and coordinating the complex care journey of patients receiving emerging therapies including genetic and other novel therapeutics, from referral through long term follow-up.
• Collaborative work with a multidisciplinary team to support payor authorization, patient and family education, coordination for inpatient and outpatient care, and other ancillary services including but not limited to; transfusions medicine, apheresis, fertility preservation and psychosocial support.
• Actively participating, as required, in hematology, oncology, stem cell transplant, gene therapy, neurology and more clinic/hospital, departmental and multidisciplinary programs, committees and special projects.
• Working 40 hours per week: days. Onsite position but can be flexible with remote for meeting specific clinic needs.

To qualify, you must have:

• A BSN; Master's in a related Healthcare field preferred.
• A minimum of 5 years nursing experience required; pediatrics preferred.
• A certification as a Certified Case Manager is preferred, if not must obtain within the first two years of employment.
• Experience as a Case Manager (Discharge Planning and or Utilization Management) preferred.
