The Nurse Practitioner or Physician Assistant will establish a patient panel of services, inclusive of patient care within assigned Northeastern Rural Health Clinics facilities. There are opportunities in the walk in clinic setting as well as in suites in any of our current or future locations. There are weekday and weekends a well as variable shift assignments.
Performs direct primary care, including diagnosis, treatment, referral, and follow up, as needed. The Nurse Practitioner/Physician Assistant will be assigned to Supervising Physicians wherein care will be overseen, co-management, observes, supports, advises and consults on patient care.
Attention to building trust in the community and the population we serve is paramount. Focus on patient care practices tailoring to the Federally Qualified Health Clinic (FQHC). The Nurse Practitioner or Physician Assistant reports directly to the Medical Director.

Duties and Responsibilities
Serve in this role as a Nurse Practitioner or Physician Assistant and will see patients in all age groups and serve as a teammate to others in Clinics
Under supervising physicians, conducts delegated duties within a program of family practice care with responsibility for examination, diagnosis and treatment of pediatric, adult, and geriatric populations
Secures case history and related pertinent information
Examines patient, utilizes laboratory and x-ray diagnostic tests, interprets findings for differential diagnosis; diagnoses ailments; and performs necessary treatment including surgical procedures, carrying out such follow-up treatment as may be medically indicated
Prepares health care lists, setting priorities and creating action plans in consultation with team members
Orders, performs, reviews laboratory tests as needed
Recommends referrals to specialists for patients needing advanced and specialized care
Participates in preventive health programs such as immunization, disease detection, disease prevention, special health screenings and Northeastern Rural Health Clinics' public health programs, i.e., Family Planning, birth control, etc.
Refers patients with serious conditions to hospital for inpatient care or refers for consultations to medical, surgical or other adjunct services
Prescribes and/or dispenses required medication to patients following the Delegation of Services Agreement and Guidelines, Protocol and Formulary Addendum, California law, NRHC policies, etc.
Seeks advice from supervising physicians and other medical specialists, as required
Supports others in the care of complicated medical problems or complications which arise out of their care
Attends to emergencies as needed
Prepares and maintains patient records in electronic health records completing charts timely, including monitoring the transfer of records to referral sources
Provides professional guidance and supervision to the assigned staff or nurses and other professionals
Participates in on call activities, works at other assigned locations, conducts rounds, and provides care throughout the community, as needed
Serves as a mentor and advisor to others
Supports all phases of family practice
Maintains licensure and certification
Responsive to supervisory feedback, utilizing constructive criticism in a positive manner
Maintains open communication and observes chain of command
Works well with peers and other employees as a team player
Sensitive to customer service issues, displaying a positive attitude to patients, co-workers, and the organization as a whole

Qualifications
Experience and Education
Nurse Practitioner
- Graduation from a recognized Nurse Practitioner's Program from an accredited school
- Current national certification as a Nurse Practitioner
- Current unrestricted licensure to practice medicine in the State of California as Nurse Practitioner
- Controlled Substance education as required by California
- Current BLS CPR certificate
- Current DEA registration
- Must meet current medical continuing education requirements, at all times
- Must possess current valid driver license and vehicle liability insurance
Physician Assistant
- Graduation from an accredited school
- Certification by California State Board of Medical Examiners as a Physician Assistant
- Current national certification/licensure as a Physician Assistant through NCCPA
- Current BLS CPR certificate
- Current DEA registration
- Controlled Substance education through California
- Must meet current medical continuing education requirements, at all times
- Must possess current valid driver license and vehicle liability insurance
