SUMMARY: SUMMARY: The Director of Quality and Care Continuum is responsible for Care Continuum, Performance Improvement (PI), Clinical Documentation Improvement, and Payor Contracts. The primary responsibilities include leading the organization's continuum of care and clinical documentation improvement initiatives from a strategic and operation perspective by identifying care coordination and documentation gaps, developing strategies for implementation to close such gaps and working collaboratively with a multidisciplinary team to meet these goals. This position works in collaboration with a multidisciplinary team to develop and coordinate comprehensive, timely, safe, cost-effective, and quality healthcare services throughout the continuum of care. In addition, this position has responsibility for coordinating the activities of the Performance Improvement Department, including data abstraction, analysis, reporting and performance / process improvement. Also included with the PI Department is the regulatory readiness program to ensure The Joint Commission (TJC), Federal and State (Centers for Medicare and Medicaid Services) compliance. Services as a resource for hospital wide and system-wide performance improvement activities.
EXPERIENCE: 8-10 years clinical experience. 4-6 years hospital leadership in supervisor, coordinator, or management position. Minimum 5 years full time involvement in hospital quality and process improvement and regulatory readiness. Preferred: Case Management experience. Working knowledge of alternative payment methodology, insurance coverage, post-acute providers as well as community resources.
SPECIAL SKILLS: Ability to facilitate effective meetings. Demonstrated knowledge of quality measurement and reporting in a healthcare setting. The ability to analyze complex data, systems, and problems, and determine possible solutions. The ability to display and analyze time series data using statistical tools. Demonstrate knowledge of quality improvement principles, including PDSA, DMAIC, LEAN, process management. Evidence of successful implementation of continuous quality and process improvement principles and practices is required. In depth knowledge of regulatory requirements including TJC, CMS, and stated with the ability to lead and influence others throughout the organization to work collaboratively to ensure continuous compliance. Excellent verbal and written communication skills, engaging in direct, open, honest interactions and feedback with a willingness to listen, teach and learn. Must be self-motivated and work independently without close supervision. Must possess the ability to work well with other professionals and staff in a creative and challenging work environment.
OTHER: Demonstrate technical competence with MS Office Suite.
LICENSE/CERT: Required: RN (Registered Nurse) State of Missouri Preferred: Lean Six Sigma Certification
