Riverside is currently seeking a full time Diabetes Nurse Practitioner to join our team at Riverside Endocrinology & Diabetes Specialists located in Newport News Virginia.
In this role we are seeking an experienced Nurse Practitioner with 3 or more years of diabetes management experience.

We're looking for an experienced Nurse Practitioner who can work collaboratively with our Endocrinology team to deliver consistently positive outcomes for our diabetic patients. You will:

Responsibilities:
The Diabetes Nurse Practitioner will provide specialty-level diabetes care in the outpatient clinical setting. Candidates with 3 years or more of experience in an acute care and/or physician practice setting with diabetes patients are strongly preferred. Candidates with strong internal medicine backgrounds and focus on endocrine/diabetes management will be considered. Position is outpatient based with a Monday-Friday schedule covering both our Newport News and Williamsburg locations.
Candidate will have one year to acquire the Advanced Diabetes Management (BC-ADM) or Diabetes Care and Education Specialist certification

Education
Must be a graduate from an accredited nurse practitioner or physician assistant program approved by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA).
Nurse Practitioner- Masters in Nursing
Nurse Practitioner License
Required
Meet comprehensive and professional standards.
Have a current, valid state of Virginia medical license or be eligible for one
Have a valid DEA controlled substance registration.
Provide Professional References from both academic and work peers.
Current ACLS/BLS certificate
Two (2+) or more years of Cardiothoracic Surgery experience as a board-certified, licensed Physician Assistant or Nurse Practitioner.
Skills Required
Strong oral and written communication skills
Collaborative attitude
Ability to work various shift in a fast-paced environment.
Ability to incorporate evaluator feedback to improve performance.
Meet preset timelines for program completion.
Basic computer skills in Word, Excel
Dedicated team member who is flexible and self-motivated.
Documentation in electronic health record - Epic
Scope of Practice
Perform physical examinations and assessments of health with a knowledge of normal versus abnormal findings.
Develop disease specific patient care plans.
Order, interpret lab and radiologic diagnostic studies.
Prescribe appropriate medications/dosing ranges.
Complete timely, accurate documentation regarding plan

- Provide exceptional direct patient care, including
- In-person and virtual clinic visits, focusing on diabetes management
- Patient phone calls and messages;
- Remote data analysis;
- Panel-level analysis to develop care plans for high priority patients
- Call responsibilities, shared with other members of the clinical team
- Maintain constructive, trust-based relationships with local primary care physicians and other members of the team
- Engage with other members of the team in formal and informal ways to improve the health of specific patients and the overall panel
What you need to be successful
- Must be a graduate and certified from an accredited advance practice program
- Three or more years of clinical experience, with a demonstrated ability to work autonomously; capable of providing care without physician supervision
- Prior work in diabetes is required; a huge plus if you are a CDCES or ADM board certified
- Must hold current, unrestricted State license or be eligible for licensure
- Comfortable with Electronic Medical Record (EMR) systems
