About You:
Role Summary:
CalAIM is California's Medi-Cal transformation initiative, moving Medi-Cal towards a population health approach that prioritizes prevention and whole-person care. CalAIM offers Medi-Cal enrollees coordinated and equitable access to services addressing their physical, behavioral, and social support needs. The Associate Director oversees the implementation and management of the CalAIM programs which includes Enhanced Care Management (ECM) and Community Support (CS) services. This role assesses organizational capabilities, identifies service gaps, works with leadership to develop an agency wide strategy and ensures successful program participation. The AD integrates ECM and CS services into current offerings, enhancing the delivery of comprehensive care to high-need populations, while also identifying new opportunities to add services.
The AD will coordinate internal and external stakeholders through the planning and implementation of the CalAIM Program. Responsibilities include partnering with local Managed Care Plans, the Department of Public Health, the Human Services Agency, and community-based organizations; identifying process and policy solutions; coordinating with Health Center staff and Human Resources for recruitment; and monitoring project workplans and dashboards.
The AD will launch and scale new initiatives, develop processes, procedures, training, and documentation, and supervise care management teams. The AD will lead the expansion of CalAIM ECM and Community Supports, developing cross-referral networks, resource databases, and outreach strategies.

Key Responsibilities:
- Supervise and oversee daily operations of CalAIM, including staffing, scheduling, and coordination services.
- Conduct regular needs assessment for the agency and build capacity of various departments to implement ECM and CS programming
- Monitor and evaluate ECM program performance, using data to identify improvements.
- Supervise and support ECM Care Managers & Community Health Workers, ensuring optimal client support and outcomes.
- Ensure outreach procedures meet enrollment targets.
- Maintain strong relationships with community partners and stakeholders, advocating for clients with complex healthcare conditions.
- Manage new client intake and streamline referral processes.
- Monitor implementation of Care Plans and recommend updates.
- Provide oversight of Care Manager assessments and interventions, ensuring quality assurance.
- Conduct regular audits to ensure compliance with documentation requirements and clean billing submissions to avoid payment delays
- Monitor caseloads to ensure client safety and program compliance.
- Coordinate all CalAIM activities, including reports, budgets, workplans, staff recruitment, and communications.
- Attend regular meetings with Health Plans (MCP) and track deliverables.
- Assess program efforts, efficiencies, and effectiveness to determine goals and priorities.
- Coach subordinates to improve performance.
- Evaluate and implement new procedures within CalAIM.
- Develop and implement operational policies for efficient program operation.
- Act as a liaison between ECM staff and MCP for program compliance.
- Ensure timely and accurate processing of ECM claims.
- Plan and facilitate staff meetings, team activities, and training.
- Provide crisis intervention as needed.
Qualifications:
- 4+ years of experience working in a multi-disciplinary community health care or other social service setting managing a staff and coordinating programs providing various levels of care including housing, medical, case management, behavioral health, and more.
- Operational knowledge of laws and regulations governing Medicaid/Medi-Cal and related public health programs.
- High level of proficiency designing, and presenting various reports such as budgets, project plans, performance evaluations, grant performance analysis, risk and benefit assessments of program implementation, etc.
- Demonstrated experience leading a diverse team of front-line workers who regularly engage with clients from vulnerable populations.
- High level of proficiency in research and network to find and comprehend community-based resources and support services and how they fit into SFCHC's goals.
- Must obtain TB clearance (within past 12 months) and complete background check upon hire.
- Current with vaccinations including latest Covid-19 vaccine with at least one booster, and seasonal flu, and able to provide proof.
Preferred:
- Advanced degree in public policy, public administration, social work, or a related field.
- Bachelors degree in related field.
- Two years of Federal or State grant management experience, including budgeting and monitoring.
- Familiarity with CalAIM, Whole Person Care, and other Medicaid Waiver programs.
- Understanding of community-based resources and support services.
- Ability to manage professional relationships and encourage team building.
- Knowledge of the EPIC Electronic Medical Record System.
- Experience working in an FQHC or other type of Community Health Center.
