  • JOB SUMMARY
    • Responsible for facilitating care and communication to all disciplines to best serve the patient that is acutely deteriorating and ultimately improve response to treatments and overall outcomes. Assessment Consultation nurses have a role in mentoring and coaching new and less experience staff in a non-disciplinary way, encouraging collegiality and teamwork using clinical expertise. Assists ICU as needed, with patient centered tasks and serves as liaison for nurses, physicians, families, and patients for those critical care patients
  • DUTIES AND ESSENTIAL FUNCTIONS
    • Responds timely to nursing department consultations of patient condition deteriorations.
    • Overall performance rating of below indicated essential job duties:
    • Utilizes the nursing process to assess, prioritize, plan and implement and evaluate necessary measures to improve patient condition and documents process.
    • Collaborates with attending physicians and utilizes approved protocols as necessary.
    • Serves as mentor to bedside nurses to improve overall patient care by supporting his/her efforts, recognizing his/her abilities and strengths, and teaching/training/educating as necessary.
    • Round daily on all patients transferred out of the ICU within 12-24 hours of transfer as well as all charge nurses to proactively identify and patient at risk for deterioration.
    • Utilizes organization-wide safe medication practices including, but not limited to the 5 rights of medication administration, barcode scanning and other medication safety processes specific to the unit/department. Knows actions and usual dosage, indications and side effects of medications administered.
    • Provides teaching and educational material to the patient and/or significant other as needed according to the care, treatment services and identified risks specific to the patient and documents patient and/or family understanding.
    • Maintains ACT call log to assist in staffing prediction as well as assist in identifying educational opportunities for nurses on the units.
  • EDUCATION/LICENSE/CERTIFICATION/OTHER REQUIREMENTS
    • Minimum requirement:
      • Graduate from an accredited school of nursing for Registered Nurses.
      • Maintains active RN license in the Commonwealth of Kentucky and any other states as applicable.
      • Achieves and maintains American Heart Association (AHA) Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), and Pediatric Advanced Cardiac Life Support (PALS).
      • Achieves and maintains Non-Violent Crisis Intervention (CPI)
      • 2 years of Critical Care Experience.
      • Completes all required training and competencies as required.
    • Preferred qualifications:
      • Bachelor's Degree in Nursing
      • Critical Care Nursing Certification (CCRN).
  • WORKING ENVIRONMENT
    • Constantly works near moving mechanical parts
    • Constantly exposed toblood and/or body fluids andinfection.
    • Occasionally exposed to toxic or caustic chemicals
    • Occasionally exposed to risk of radiation
    • The noise level in the work environment is usually moderate.
  • PHYSICAL DEMANDS
    • Occasionally required to maintain in a stationary position (standing or sitting).
    • Frequently required to walk and move about.
    • Frequently required to operate a computer and other equipment.
    • Frequently required to use fine motor skills to handle and operate instruments/devices.
    • Constantly required to lift and/or move items up to 10 pounds.
    • Frequently required to lift and/or move items up to 50 pounds.
    • Occasionally required to lift and/or move items up to 100 pounds.
    • Constantly required to communicate with patients and other care team requiring talking and hearing.
    • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception.
    • Occasionally required to position self in order to meet patient needs.
