Job Description
PURPOSE OF POSITION
The Licensed Social Work Level IV is an LCSW who works independently as well as in collaboration with transdisciplinary teams, to identify psychosocial issues and concerns of patients and their families and provide appropriate clinical services to meet their needs. They function as a primary therapist in the Partial Hospitalization Program under the Adult Outpatient Mental Health Clinic servicing individuals with severe mental health conditions and co-morbid medical and substance abuse conditions. They are responsible for monitoring the performance of direct report and completing annual performance appraisals
AREAS OF RESPONSIBILITIES
Licensed clinician with a minimum of 3 years clinical experience working with the severely mentally ill, chemically addicted and intellectually disabled population who is responsible for providing individual and group therapy to patients in the Partial Hospitalization Program. Candidate should have at least 1 year of supervisory experience that include conduction annual performance appraisals and managing time and leave requests for direct reports. The candidate should have crisis management skills, and aftercare planning skills for the population served. Candidate must be skilled in delivering individual and group treatment to individuals discharging from an acute care setting or who require a higher/intensive level of outpatient treatment. Candidate should be able to manage their caseload, new intakes, be familiar with resources, address concrete needs of clients and organize self around such. Additional areas of responsibility include managing and assessing staff training needs, schedule and coordinate intakes received by the clinic, if needed.
SUMMARY OF DUTIES AND RESPONSIBILITIES
The duties include providing clinical and administrative supervision to direct reports that include licensed and non-licensed Social Work staff. The role requires clinician to conduct initial psychiatric assessment, deliver individual and group therapy, document biopsychosocial assessments, treatment planning, formulation and disposition. The clinician may be responsible for managing intakes for the outpatient department, developing comprehensive treatment plans for patients assigned to them, writing discharge summaries and other related administrative work, based on program needs. Other duties include facilitating supportive, psychoeducational and insight-oriented psychotherapeutic interventions, providing crisis interventions to high-risk patients, engaging with and providing support to families, and engaging them as partners in recovery. Identifying and collaborating with existing support systems, facilitating community referrals for patients as needed, collaborating with interdisciplinary treatment team to develop most appropriate case disposition, documenting all interventions within the required regulatory timeframes is required of the clinician. The clinician is responsible for providing direct supervision to licensed and non licensed Social Work staff, providing support to other team members and managing case assignments in the program. They are responsible for developing workflows and contribution to policies when indicated
Good writing skills and ability to work in a fast-paced environment required. English proficient a must; ability to communicate in Spanish or Creole a plus.
Minimum Qualifications
1. Valid license as an LCSW issued by the NYSED and one (1) year of supervisory experience utilizing casework, group work and/or community organization methodologies.
