What you will do:

• Provide embedded specialty care in Federally Qualified Healthcare Centers (FQHC) to increase access for community members in places they already know and trust.
• Improve health outcomes
• Reduce health disparities by eliminating barriers to healthcare that disproportionately impact under-resourced communities.
• Work within Community CARE, a circle of programs that exists to honor the human dignity of all by building connections that expand the reach and definition of healthcare.
• Provide holistic care by coupling medical care with dedicated support services in the areas of nutrition, clinical case management, and social determinants of health.

What you will need:

• IL medical license or pending
• DEA/Controlled Substance License or pending
• Education: Gastroenterology Fellowship
• Certification: Board Certified or Board Eligible
