Reporting to the Pharmacy Operations Manager, the Pharmacist in Charge practices pharmacy in a community setting with clinical functions. Responsible for day-to day pharmacist in charge duties.

Overview
Reporting to the Pharmacy Operations Manager, the Pharmacist in Charge practices pharmacy in a community setting with clinical functions. Responsible for day-to day pharmacist in charge duties.
Essential Functions
- Regular communication with Senior Director of Pharmacy to optimize MHF pharmacy services.
- Assume Pharmacist in Charge (PIC) designation with the California Board of Pharmacy.
- Assume responsibility for pharmacy medication inventory including ordering and return of expired CII drugs in accordance with state and federal laws.
- Implement pharmacy processes as directed by the Senior Director of Pharmacy.
- Attendance, participation, and leadership of committee meetings as assigned (ex. Provider team, Pharmacy and Therapeutics, Safety, Multi-disciplinary team, Research)
- Prescription processing and verification, coordination of prescription delivery, insurance claim transmission issue resolution, and workflow maintenance.
- Perform drug utilization reviews of patient profiles to prevent drug-drug interactions, drug-disease interactions, and duplicative therapies, and to increase medication adherence.
- Patient counseling on all aspects of medication use in accordance with state and federal laws.
- Clinical pharmacy services such as: immunizations, Medication Therapy Management (MTM), smoking cessation, etc. and maintain the associated certifications.
- Prescriber consultation to ensure appropriateness of therapy and medication safety.
- Communicates with other program staff including the Department of Health, CQM, etc.
- Documentation of all patient counseling, clinical encounters, and coordination of patient care in appropriate record.
- Answer pharmacy phone calls, return calls to patients.
- Travel for training and patient care.
- Understanding and compliance with 340B program, DEA, Board of Pharmacy, patient assistance programs (internal and external).
- Serves as laboratory director for CLIA waiver.
- Maintenance of a work environment conducive to excellent customer service.
- Participate in regularly scheduled and ad hoc pharmacy inventory as directed.
- Maintenance of a clean and organized pharmacy environment.
- General knowledge of and ability to perform technician duties in the case of an absence or crisis, including opening/closing procedures.
- Provides guidance for clinical operational aspects of all programs [CPE 3-2 e]
- Provides oversight of all clinical decision-making aspects of all programs [CPE 3-2 c]
- Ensures that qualified clinicians are accountable to the organization for decisions affecting consumers [CPE 3-2 d]
Qualifications
- Two years of relevant pharmacy experience.
- Doctor of pharmacy degree or equivalent with at least one year related experience and/or training.
- Valid Registered Pharmacist license from the California Board of Pharmacy
- Bi-Lingual in Spanish (Preferred)
- Completion of a PGY1 Pharmacy Residency
- AAHIVM Certification within 3 years of hire
- Specialty experience in HIV, Hepatitis C, and/or pain management
- CPR certification
- Certification in immunization delivery
- Participate in annual OSHA training and HIPPA training
- Power of Attorney for CII ordering obtained upon hire
- Ability to prioritize effectively and meet strict deadlines while managing multiple, competing tasks in a fast-paced organization.
- Strong team player able to form functional work relationship with peers and superiors.
- Ability to perform other duties as assigned.
- Knowledge of Microsoft Office Suite; Microsoft Word; Microsoft Excel
- Basic operation of a computer workstation, electronic medical record, and pharmacy computer system.
- Use of Windows Explorer (electronic file-handling).
- Computer-related problem-solving skills through the use of available trainings and help desk.
- A record of satisfactory performance in all prior and current employment as evidenced by positive employment references from previous and current employers.
