Responsibilities
Summary:
Provides direct medical management including diagnosis and management of acute and chronic diseases. Services are coordinated in concert with Attending Physicians, of patients cared for by adult intensive care and pulmonary service
Responsibilities:
- Uses scope of role to provide the direct medical management using clinical reasoning which includes clinical decision-making, critical thinking in the care of patients with acute and chronic disease process.
- Support the Adult Critical Care and Pulmonary inpatient service, as required, rotating through night and day shifts. Remains in-house during assigned shift.
- Work with the Attending Physician in completing admissions and consultations, rounding, and managing patient care during the assigned shift.
- Provide complete and timely dictation on all clinical work. Submits patient billing paperwork, as directed.
To order, and analyze data from multiple sources to formulate differential diagnosis, plan of medical management and monitor and evaluate outcomes. - Establish the plan of care in collaboration with the family, Attending Physician, staff nurse and other health care team members to ensure continuous monitoring and evaluation of said plan from admission through discharge.
- Utilize prescriptive privileges.
- Utilize optimal communication skills with patients, families, and all healthcare providers and staff.
- Perform high risk skilled procedures required for Critical Care Medicine Clinical Privileges. Requests and maintains said privileges with Medical Staff Affairs, within 12 months of start date.
- Facilitate the transport of critically ill patients from surrounding communities to the Presbyterian facility.
- Uses scope of role to act supporting the patient/family in facilitating meeting their concerns in resolving ethical and clinical issues.
- Integrates a compassionate, supportive and therapeutic environment for patients/families while supporting an exceptional patient experience.
- Involves the use of the multidisciplinary team as well as the patient to promote optimal and realistic goals/outcomes.
- Maneuvers and manages the environment and system resources to meet patient/family needs.
- Incorporates mind, body, spirit approach in provision of patient centered care by recognizing and incorporating cultural, spiritual, gender, race, ethnic, lifestyle, socioeconomic, age and value differences in care decisions.
- Fosters patient/family learning to promote self-care using formal and informal process as applicable and at the appropriate level of understanding.
- Participates and fosters the educational development of colleagues through formal and informal interactions.
- May serve as a preceptor to graduate nurses or physician assistants and new hires.
- Uses evidence-based practices standards in all aspects of care.
- Participates in the collection of data, quality assurance analysis and on-going evaluation of the effectiveness in established standards of care.
- Supports shared governance by participating in job related projects which may include research, policy and policy / procedures, or program development to improve the quality of patient care.
- Performs other functions as required.
