Job Description

PURPOSE OF POSITION
Under direction, formulates, plans, directs, coordinates and evaluates pharmacy services and programs in assigned Gotham Health facilities, and promotes effective functioning and operations of pharmacy in coordination with other professional and administrative disciplines.

AREAS OF RESPONSIBILITIES
Oversight for all aspects of pharmacy services and programs within the assigned facility/facilities, including supervision of pharmacy staff.

SUMMARY OF DUTIES AND RESPONSIBILITIES
1. Enter into collaborative practice agreement with physician(s)/ physician group(s) and provide direct Collaborative Drug Therapy Management.
2. Develops, recommends and implementation of pharmaceutical service programs consistent with the medication needs of the patients, including control and accountability of drug products including requisitioning and dispensing of pharmaceutical supplies to nursing care units and to other departments/services of the hospital.
3. Participates with hospital medical staff in formulation, development and recommendation of policies regarding the evaluation, selection, procurement, storage, distribution, use and all other matters relating to drugs in the hospital.
4. Assists in formulating, developing, implementing and directing clinical pharmacy activities involving the review and interpretation of all drug orders, maintenance of patient medication profiles and participation in direct patient care.
5. Assists in the development and preparation of department budgets and makes recommendations regarding staffing
patterns and pharmacy facilities.
6. Directs pharmacy personnel with respect to compounding and dispensing of drugs in accordance with prescribed
policies and standards and legal requirements, requisitioning and procurement of drugs and pharmaceutical
supplies, inventory control and maintenance, storage of supplies and their issuance, and maintenance of such
controls and standards, orderliness and cleanliness of pharmacy area, and inspection programs.
7. Recruits, selects, assigns and evaluates Pharmacy staff.
8. Develops and evaluates continuing in-service training programs for pharmacy, nursing and medical staff with
respect to drug therapy.
9. Participates in educational programs with medical and nursing staff relating to drug usage, potential drug
interactions and drug incompatibilities.
10. Participates in those aspects of hospital's patient care evaluation programs relating to drug utilization and
effectiveness.
11. Participates in all meetings of the pharmacy and therapeutics committee and implements the decisions of the
committee throughout the hospital.
12. Maintain liaison relationships with medical and nursing staff; provide timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls, and Joint Commission Standards.
13. Works as part of a team to provide comprehensive, personalized care for all New Yorkers and commit to providing a person-centered care environment while exemplifying ICARE (Integrity, Compassion, Accountability, Respect and Excellence) values.
14. Perform other related duties incidental to the work described herein.

Minimum Qualifications
1. Valid license and current registration to practice as a Pharmacist issued by the New York State Education Department (NYSED); and, either 2 or 3 below;
2. Eight (8) years of satisfactory, full-time experience as a licensed practicing Pharmacist, of which at least four (4) years shall have been in an administrative, managerial and supervisory capacity in a Pharmacy in a hospital, health or medical care facility or
3. Possession of a Master's degree in Hospital Pharmacy Administration or a related discipline or completion of NYSED accredited Pharm D program, and six (6) years of satisfactory, full-time experience as a Licensed Pharmacist, of which at least two (2) years shall have been in an administrative, managerial and supervisory capacity in a Pharmacy in a hospital, health or medical care facility.

QUALIFICATIONS FOR THE JOB:

CERTIFICATION(S)/LICENSE(S):

EDUCATIONAL LEVEL:
1. Graduation from an accredited college of Pharmacy; and,
2. Possession of current New York State Pharmacy license; and,
3. Four years of satisfactory full-lime experience as a Licensed Practicing Pharmacist, of which at least two years shall

have been in an administrative, managerial and supervisory capacity in Pharmacy in a hospital, health or medical ability; or,
4. A satisfactory equivalent combination of education, training and experience.

Epic EMR, Microsoft Office Suite

YEARS OF EXPERIENCE:
2 - 5 years' experience in supervisory role is preferred but not required

KNOWLEDGEABLE IN:
1. Ability to manage and direct the work of individuals in different titles to improve patient care.
2. Ability to utilize drug information resources and provide appropriate information.
3. Ability to communicate effectively.
4. Ability to manage inventory and control spending and collecting data and prepare reports.
