Harlem Hospital Center has been a bedrock of the Harlem community since 1887. The hospital provides a wide range of medical, surgical, diagnostic, therapeutic and family support services to the residents of Central Harlem, West Harlem, Washington Heights and Inwood. Duties & Responsibilities
SUMMARY OF DUTIES AND RESPONSIBILITIES:
Purpose of Position: This class of positions encompasses professional staff, supervisory, clinical and administrative work within a pharmacy department, with varying degrees of latitude for independent initiative and judgment. Health System Pharmacists adhere to Federal, State and local laws and other regulatory requirements regarding pharmacy and institutional practice. There are three (3) Assignment Levels within this class of positions, and all personnel may be required to perform related work. The following are typical assignments within this class of positions. Assignment Level I Under direction, with moderate latitude for the exercise of independent initiative and judgment, provides professional pharmacy services in a hospital, clinic or other health care setting. Assignment Level I:
Examples of Typical Tasks:
1. Fills prescriptions and patient orders, resolves discrepancies in patient therapy, and follows-up on problems.
2. Utilizes clinical data in evaluating physician orders to ensure appropriate therapy.
3. Provides and documents drug information and patient counseling.
4. Documents interventions, adverse drug reactions (ADRs), and medication errors.
5. Directs and utilizes personnel including technicians, aides and clerical staff.
6. Assists in the monthly inspection of the patient care units and the Pharmacy.
7. Maintains adequate medication supply in individual work area.
8. Participates in Pharmacy staff in-services and meetings relating to Systemwide and/or health care setting programs.
9. Operates and troubleshoots automated dispensing systems, as needed.
10. Advises prescribers on alternatives for non-formulary drugs.

For Assignment to Level I 1. Possession of a valid NY State license to practice pharmacy.
