Qualifications:
- Registered professional nurse, licensed to practice nursing in New York State
- One to two years of recent bedside nursing experience with good clinical skills
- At least two years recent Med-Surg. nursing experience
- A Bachelor's degree is strongly preferred. BSN a plus
- Some experience caring for terminally ill patients and a basic understanding and commitment to the hospice concept
- Experience in home care, hospice, and/or oncology preferred
- Ability to relate to a diverse population and workforce in a personable and effective manner
- Ability to assess the needs of terminally ill patients and their families and to plan care to meet these needs
- Basic knowledge of medications, treatments, and therapies for pain and symptom control
- Ability to communicate effectively orally and in writing; good listening skills
- Ability to work within a team concept
- Must be proficient in Microsoft Windows
- Experience with EMR system
- Must have access to a vehicle, a valid driver's license, and current insurance
- Actively participate in the coordination of all aspects of patient care, per current professional standards and practice
- Ability to develop, evaluate, and provide direct care according to the established patient plan of care and delegate to others
- Ability to carry out a variety of treatments, procedures, and examinations
Responsibilities:
- Meets with patient/family at home, in an inpatient facility to explain UH services and assess patient/family needs
- Contributes to patient and family counseling and education explaining needs at the initial visit and explaining the UH philosophy, services, and advance directives.
- If the admission is from an inpatient facility, work with hospital discharge planners to effect timely and coordinated patient discharge.
- Conducts assessment of patient/family
- Conducts admission which includes:
- Obtaining demographic information, medical information, and history, and documenting all necessary information
- Performing physical assessment and pain assessments, obtaining necessary consents and authorizations, preparing the initial nursing care plan, and preparing the HHA care plan.
- Obtaining initial medical orders from the primary MD and informing MD of UH admission.
- Assists Intake Department with referral, computer input as needed
- Assists with additional admissions procedures as needed
- Participates in weekend rotation as appropriate
- Accepts other responsibilities as assigned
