Job Description:
  • The advanced practice nurse shall perform consultation visits for palliative care, goals of care meetings, symptom management visits, or Hospice F2F visits at the request of the licensed practitioner.
  • Example of other services that can be performed and are in the APC's scope of practice as per his/her job description and documented competencies include, but are not limited to, performing physical examinations and medical histories, ordering laboratory exams, conducting diagnostic and therapeutic procedures, prescribing appropriate interventions and medications, providing health promotion and safety instructions, managing acute episodic illness and stable chronic diseases, and making referrals to other health care providers, as needed.
  • The Supervising Physician will be responsible for providing ongoing supervision, collaboration, consultation, and evaluation of the medical acts performed by the APC as defined in this Agreement, and will retain responsibility for the care rendered by the APC. If the Supervising Physician is not available an alternate Supervising Physicians will serve in this capacity. The APC will maintain the Collaborative Agreement and state documents current with all Alternate Supervising Physicians.

How coverage will be provided during periods of absence, incapacity, infirmity, or emergency by the license practitioner:

Scope of Practice and a Description of Limitations
  • The APC will function in accordance with the established scope of practice as specified in his/her job description, in accordance with Applicable Home Health, Palliative and Hospice Rules, and following all licensing regulations.
  • Consultation by the APC with the Supervising Physician or Alternate Supervising Physician shall be required whenever:
  • 3.Prescriptive Authority: The APC will adhere to the Applicable Rules, including, but not limited to, the Provider Prescription Ordering Policy, and will maintain all licenses required for Prescriptive Authority by the Applicable Rules, as well as an active DEA registration. The APC can prescribe appropriate medications in accordance with the established scope of practice, this Agreement, and the Applicable Rules.
  • Time and Manner of the Licensed Practitioners Review of the Advanced Practice Nurse Prescribing Practices: The advanced nurse must submit all documentation within the hospitals system's guidelines but no later than seven days for the licensed practitioner to review. The review of prescribing practices shall include a five (5%) percent random sampling of records including reviewing the medications prescribed for patients.
  • Procedures or Other Patient Care Activities: It is the obligation and responsibility of the APC to verify that the procedures/competencies are performed in compliance with all Applicable Rules, including, but not limited to, physician supervision requirements.
  • Ordering Medical Devices, Biologicals, Respiratory Therapy, Physical Therapy, Home Health, Orthotics and Prosthetic and Hospice: The APC may order medical devices, biologicals, respiratory therapy, physical therapy, home health, orthotics and prosthetics and Hospice when appropriate and in accordance with the established scope of practice as specified in his/her job description and in accordance with the applicable rules.
  • No other limitation are being placed on the advanced practice nurse
  • Medical Records: The APC will be responsible for the complete documentation of all patient encounters, consistent with Applicable Rules. APC's encounters should be completed and closed by end of the business day in which such encounters occurred, but not to exceed 3 calendar days of the encounter, and in accordance with Dependable policies.
    • a)Deviations from protocol and/or guidelines
    • b)Clinical practice issues are encountered; and/or
    • c)Patient's condition acutely deteriorates, or a medical emergency arises.
