JOB SUMMARY:
This position is responsible for oversight of day-to-day medical clinic operations, including supervision of registered nurses and medical assistants, supply management and managing patient feedback. Works collaboratively with Site Medical Director and Operations Director to coordinate smooth clinic functioning. Oversees and participates in the delivery of direct and telephonic nursing services to patients and families in accordance with the scope of the RN. Able to plan, provide and document professional nursing care in ambulatory settings, utilizing the nursing process, in accordance with physician orders and established policies and procedures. Delegate tasks and supervise the activities of other licensed and unlicensed care providers.

MAJOR DUTIES AND RESPONSIBILITIES:

Work Group Effectiveness
Ensures that staff appropriately and professionally delivers the quality of care as determined by CBHC standards
Encourages and provides continuing education opportunities for staff

Strategic Leadership
Delegates clerical and non-nursing tasks to assistive personnel.
Supervises daily work of nursing team through direct observation and periodic chart reviews

Performance Management
Provides appropriate feedback on performance

Communication
Meets weekly with other site leadership and monthly with Lead RNs
Documents both incoming and outgoing calls as they occur and communicates effectively with other team members to provide continuity of care to the patient
Utilizes interpreter services for patient communications
Attends departmental and all staff meetings.
Engages in Care Coordination both internal to and external to our organization

Checking, Examining & Recording
Documents telecommunications that reflect care specific to the actual or potential health needs of the caller/patient.
Documents and communicates plan of care
Completes assessment of symptoms

Planning and Organizing
Locates and utilizes appropriate resources inside and outside the organization to meet the needs of the caller/patient
Schedules patient visits based on acuity of need
Develops appropriate staffing schedules for appropriate clinic coverage.

Integrity
Maintains confidentiality in all situations

Compliance, Policy, and Procedure
Adheres to nursing protocols and organizational standing orders

Analytical and Critical Thinking
Participates in patient appointments as needed for education or treatment of disease
Runs independent nursing visits with patients for follow up of chronic illnesses, education and treatment of disease as needed
Assists in triage of patients during walk-in clinic
Makes accurate assessments/ nursing diagnosis as observed by Supervisor and Clinicians.
Collects health status data relevant to the patient's present problem in a continuous, systematic and complete manner
Completes physical assessment and correlates with patient's health history to prepare patient for further examination or treatment as appropriate

Problem Solving and Decision Making
Uses assessment to evaluate patient needs and formulate a plan of care, revises outcomes and interventions based on results of evaluation, involves the patient in the evaluation process and collaborates with members of the healthcare team
Correlates psychophysiology screening, physical complaints with life status information to screen for somatic complaints associated with the stress of every day coping.

Interpersonal Skills
Demonstrates awareness of cultural, ethnic, gender and racial differences and their potential to impact patient outcomes

Willingness to Learn
Requests coaching and assistance from preceptors/other experienced staff when needed
Maintains membership in professional organizations such as the AAACN

Professionalism
Consistently displays professionalism in dealing with customers, staff, and clinicians as observed by manager

Teamwork
Pitches in to cover shortages within departments.
Collaborates with nutritionist, clinicians, pharmacy, mental health, addictions, case management and others in assessing patient's status
Implements plan with patient and health care team

SKILLS AND ABILITIES:
Ability to work with computers in word processing, and database applications preferred.
CPR certified within probationary period.
Must be flexible and adapt to a changing environment.
Must attend five in-services annually.

EDUCATION AND/OR EXPERIENCE:
Required: Graduate from an Accredited School of Nursing
Required: One year of nursing leadership in an ambulatory or acute care setting.
Significant related experience with populations served may substitute for above requirement.
