JOB PURPOSE:
Coordination of evaluation and screening process for patients with cardiac disease being evaluated for complex cardiac surgery. Serve as the primary point person for the valve team, including physicians, patients, and other clinical professionals. Coordination of inter-departmental processes and communication. Make recommendations to physician team for final determination. Serve as patient navigator.

KEY RESPONSIBILITIES:
1. Participate in ongoing program evaluation and program development, including coordination of inter-departmental processes and communication.
2. Provide oversight of clinic schedule and inter-departmental processes and communications including patient tracking and triage.
3. Facilitate the development and implementation of teaching plans specific to patients and to multidisciplinary staff members, including involvement in patient consent.
4. Plan, coordinate, and implement care of valve patients in the outpatient clinic.
Provide ongoing screening of patients. Assess financial, medical and social resources of current and potential valve program patients in conjunction with the research team.
Responsible for reviewing diagnostic imaging studies and lab values.
Physician Assistant, Nurse Practitioner, and Registered Nurse only: Plan, organize and provide direct care to patients. Review clinical progress and perform advanced diagnostic and therapeutic tasks.
Coordinate communications and make referrals as necessary to referring, valve, and primary care physicians along with the research team. Serve as a resource for the hospital staff regarding the care of the valve patient.
Manage waitlist and facilitate reimbursement contact.
5. Participate in evaluation and outcome monitoring of the provided clinical care.
Manage mitral regurgitation database.
Play an integral role in the investigation, development and implementation of care delivery models and valve program activities targeted to improve patient satisfaction while increasing clinical efficiency, safety, and quality.
Qualifications:
MINIMUM EDUCATION REQUIRED:
Graduate from an accredited school of nursing.

MINIMUM EXPERIENCE REQUIRED:
Demonstrated experience with and knowledge of current medical, surgical and pharmaceutical treatment modalities of the cardiothoracic surgical patient. Experience in program development, quality improvement, research, development of evidence based clinical protocols, and regulatory readiness program planning. Previous experience in professional presentations and group education.

MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Current License in the State of Georgia as a Registered Nurse or NLC/eNLC Multistate License.

ADDITIONAL PREFERRED QUALIFICATIONS:
Three years of nursing experience in cardiology or critical care.
