  • JOB SUMMARY
    • The Orthopedic Patient Navigator reports to the Clinic Manager and is responsible for coordinating the care of patients undergoing elective total joint replacement surgery. This role includes providing education and support to patients, family, staff and caregivers. The goals of this position include improving patient care and flow, eliminating barriers and improving the overall patient experience by connecting the patient and the family with appropriate resources and services. The patient navigation process includes consistent care coordination throughout the continuum of care and an assessment of the physical, psychological, and social needs of the patient.
  • MINIMUM QUALIFICATIONS
    • Education: See Licensure/Certification/Registration requirements.
    • Experience: Three (3) years nursing experience.
    • Licensure/Certification/Registration: Current license in Minnesota to practice as a Registered Nurse. BLS Certification with a designation of either BLS Provider or Healthcare Provider from either the American Heart Association, American Red Cross or Military Training Network.
  • PREFERRED QUALIFICATIONS
    • Education: Bachelor's Degree (i.e., Healthcare Administration, Business, Nursing, Education).
    • Experience: Five (5) years nursing experience. Total joint replacement experience is preferred.
    • Licensure/Certification/Registration: Orthopedic Nursing Certification
  • KNOWLEDGE, SKILLS AND ABILITIES
    • Excellent written and oral communication skills. Well-developed level of comfort working with physicians. Ability to build consultative relationship to work collaboratively and to develop successful partnerships with others. Experience in creative program developing and marketing. Ability to work successfully with staff, volunteers and community representatives.
    • READING - Intermediate: Ability to read and interpret documents such as operating and maintenance instructions and procedure manuals.
    • WRITING - Intermediate: Ability to write routine reports, correspondence, or procedures.
    • SPEAKING - Intermediate: Ability to effectively present information in one-on-one, small group situations or before groups of customers, clients, and other employees of the organization.
    • MATHEMATICAL SKILLS - Basic Skills: Ability to add and subtract two-digit numbers and to multiply and divide with 10's and 100's. Ability to perform these operations using units of American money and weight measurement, volume, and distance.
    • REASONING ABILITY - Intermediate Skills: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
    • AGE SPECIFIC COMPETENCIES: Skilled at assessment and knowledgeable of growth and development. Provides appropriate care respective to the ages of the patients served per clinical environment.

      • Typical Noise Level - Moderate noise (examples: business office with computers and printers, light traffic)
