Essential Functions and Responsibilities
- Team Leadership: Oversee the Chronic Care Management (CCM) team, including Clinical Care Specialists (CCS), ensuring effective coordination and communication across all levels.
- Operational Management: Manage the day-to-day operations of the CCM team, ensuring that patient care is delivered in a timely and efficient manner.
- Care Plan Oversight: Supervise the creation and management of individualized Care Plans for patients with chronic conditions, ensuring they are up-to-date and accurately tracked.
- Metrics & Performance Management: Monitor key performance indicators (KPIs) such as patient engagement, care plan adherence, and health outcomes to ensure the CCM team meets established goals.
- Quality Improvement Initiatives: Lead process improvement and quality control efforts to enhance the effectiveness of CCM services and patient care outcomes.
- Regulatory Compliance: Ensure that all CCM activities are compliant with applicable regulatory standards, including CMS guidelines for Chronic Care Management and Transitional Care Management.
- Training & Development: Provide ongoing training and support to the CCM team, ensuring they are equipped with the latest knowledge and tools to deliver high-quality care.
- Patient & Stakeholder Communication: Facilitate effective communication between patients, their families, and the healthcare team, ensuring that all parties are informed and engaged in the care process.
- Technology Utilization: Leverage electronic health records (EHR) and other healthcare technology to enhance care coordination, patient tracking, and documentation.
- Strategic Planning: Collaborate with senior leadership to develop and implement strategies that align CCM services with organizational goals and objectives.

Shift Structure
- Monday-Friday, 8 hour shift (Typically 9am-5pm but can vary)
Requirements and Qualifications
- Must possess an RN, NP, PA, or MD license
- Excellent communication and time management skills required
- Minimum 5 years of experience in nursing or a related clinical role, with at least 2 years in a leadership or management position.
- Strong understanding of Chronic Care Management (CCM), including regulatory requirements and best practices.
- Experience in managing remote teams and telephonic case management.
- Proficiency in healthcare technology, including EHR systems and care coordination tools.
- Excellent communication, critical thinking, and problem-solving skills.
- Demonstrated ability to lead and motivate a team, fostering a positive and productive work environment.
- Autonomous, independent worker with a strong understanding of healthcare staffing and needs.
Physical Requirements
- Must be punctual or on time and adhere to the company's Time and Attendance policy.
- Must be able to remain sitting for the majority of their shift.
