JOB SUMMARY
The Community based palliative care registered nurse (RN) serves as a professional and qualified registered nurse with the responsibility to practice their profession commensurate with their licensure, training, and experience in accordance with the laws and regulations governing their practice in the state in which services are performed, and all guidelines of applicable professional and accreditations agencies. The community based palliative care RN is responsible to work in collaboration with patients, their families and other caregivers, the patient's primary care physician, and other specialists as appropriate, to deliver episodic acute care and chronic medical management for patients with progressive illnesses under the direction of the Palliative Care Medical Provider, as directed by the plan of care and regulations of a patient's episode. The Goal of the community based palliative care RN is to support patient and family transitional care needs across care settings (ie. acute care discharges back to community setting, home health episodes, hospice, etc.), and through disease stage and changes in support of the care and/or treatment preferences established through goals of care discussions.

SPECIFIC REQUIREMENTS
Interpersonal communication (both oral and written) required to educate and comfort patients and their families. Maintain appropriate documentation records. Professional licensure required in Maryland.

REQUIREMENTS
- Required Associate's Degree 1352
- Preferred Bachelor's Degree 1352
- 2-4 years Minimum of 3 years experience working in an inpatient or outpatient health care setting required. Required
- 2-4 years Experience working as an RN in a palliative/hospice care inpatient or outpatient setting preferred. Preferred
- RN - Registered Nurse
- CPR - AHA Healthcare Provider
- CHPN - Certified Hospice Palliative Nurse
