General Description:
The Physical Therapist at Evara Health plays a vital role in our multidisciplinary team by assisting patients in developing, recovering, improving, and maintaining the skills needed for daily living and working. He/she is instrumental in providing specialized care to patients with various musculoskeletal and neuromuscular conditions. The Physical Therapist collaborates with a multidisciplinary team to ensure holistic, patient-centered care, aligning with our mission to deliver quality healthcare services.

Essential Duties & Responsibilities
- Conduct comprehensive assessments to evaluate patients' physical condition and establish personalized treatment plans.
- Implement therapeutic interventions, including exercise, manual therapy techniques, and education, to meet treatment goals.
- Monitor patients' progress, adjusting treatment plans as necessary to achieve optimal outcomes.
- Provide education to patients and families on treatment plans, exercises, and strategies to maintain and improve health and well-being.
- Collaborate with healthcare professionals across departments to ensure integrated and comprehensive care.
- Maintain accurate and detailed patient records in compliance with legal and professional standards.
- Stay current with advancements in physical therapy and incorporate evidence-based practices into patient care.
- Actively collaborates with and mentors residents, students, and trainees, integrating Evara Health's mission into clinical education, and supports clinical support staff in delivering comprehensive healthcare.
- Ensures the highest quality of comprehensive health care is provided at the lowest cost.
- Demonstrate knowledge of and compliance with regulatory agencies and established organizational policies and procedures, including strict adherence to all Evara Health, HIPAA, HRSA, State, Federal, and accreditation agency rules and regulations.

Minimum Qualifications:
- Doctorate Degree from an accredited program
- A minimum of two years of professional experience in physical therapy, demonstrating a broad range of clinical skills in a healthcare setting
- Valid and active physical therapy license
- BLS certification required
