The Advanced Illness Program at Maimonides Medical Center is seeking a Physician Assistant or Nurse Practitioner. Under the direction of the Director of Business Intelligence and Value-Based Management, the AVP of Advance Practice Providers and Social Work Director, the Program Manager is responsible for managing the day-to-day operations of the Medical Center's Advanced Illness Management program (AIM) and Age Friendly Health System Projects.

Requirements:
- Completion of an accredited Physician Assistant or Nurse Practitioner program with current NYS licensure and registration.
- Certification: ANCC or AANP or AAPA.
- ACLS and BCLS certified.
Preferred:
- Knowledge of palliative care.
- Palliative care certification.
- 1-2 years of Physician Assistant or Nurse Practitioner experience.
- Excellent interpersonal and English verbal/written communication skills.
- Must be dynamic, and have the ability to influence.
- Ability to problem-solve.
- Understanding of cost accounting concepts.
- Strong computer proficiency, especially with MSOffice (Word, Excel, Outlook).
