JOB SUMMARY: Ensures that all services are delivered in accordance with the Mission Statement and S.P.I.R.I.T. Values of Carroll Hospital Center and Carroll Hospice. The Hospice Liaison serves in a consultant role for Carroll Hospice acting as an advocate for Carroll Hospice programs and providing education for healthcare professionals, patients, and families in the surrounding community. The purpose of this role is to improve care for all patients who have a life limiting illness and specifically to facilitate admission of eligible patients to hospice care in the setting of their choice.
REQUIREMENTS: Associate's degree in Nursing requires, BSN preferred. 2-4 years of RN experience in hospice care, regulations, management, administration, marketing, education or discharge planning required. Ability to collaborate with others and to initiate and plan own work activities. Effective patient and staff teaching. Maryland Registered Nurse License, American Heart Association CPR Healthcare Provider Certification.
