Job Summary

The Director of Population Health - Employee Health Benefits provides leadership and expertise to improve care management and population outcomes for insureds covered by the employee health plan. This role will lead efforts, in collaboration with the Benefits, Population Health, and Finance teams, to innovate, operationalize, and measure the impact of care management programs within the Employee Health Plan and ensure compliance with applicable regulations. The Director will work closely with various stakeholders, including healthcare providers, to develop, implement, and sustain programs that promote preventive care and chronic disease management, ultimately enhancing the overall health outcomes of employee health plan insureds.

Essential Functions
  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • Analyzes and monitors employee health benefit data, providing subject matter expertise, for care management improvement opportunities.
  • Responsible for the development of scorecards and reports to identify and track performance of care management and population initiatives, in collaboration with the actuarial and analytics teams.
  • Leads efforts to develop plans for meeting and exceeding care management and population outcome objectives, working closely with Benefits, Population Health, and Finance to ensure alignment with employee health plan-related budgetary goals.
  • Serves as lead point of contact for executive teams related to strategizing and reporting on key care management initiatives within the employee health plan.
  • Provides insight and consultation to key stakeholders, staying current on care management and employee health benefit regulations, trends, and innovations.
  • Direct and manage multiple projects in various stages of development, implementation, and evaluation.
  • Develop & maintain strong, positive business relationships with key internal clients to clearly understand their business processes, and the development/articulation of short- & long-term business objectives.
  • Mobilize and lead cross campus and departmental project teams to meet the objectives within the given time frame, and within budget; have a strong project management mindset.


Qualifications

Competencies (Knowledge, Skills, and Abilities Required):

Legal and Regulatory Environment: Knowledge of federal, state, and local laws pertaining to care management, health data privacy, and employee health plan operations.

Budgeting: Ability to plan and administer budgets.

Project Management: Ability to lead and organize projects.

Minimum Education and Experience Requirements:

Education:

Bachelor's degree, Masters preferred

AND

Experience:

5 to 7 years in population health or care management; employe health benefits experience preferred.
