Schedule:
After your orientation period - your schedule typically be Monday to Friday, 8:00 AM to 5:00 PM.
Caseloads and Visits:
Your caseloads will be variable based on schedule and patient census. Consultation visits can be up to 90 minutes, follow up visits can range from 15 to 45 minutes.
What you'll be doing:
You'll see your patients in a hospital environment. We provide quality care regardless of circumstance to a variety of different socioeconomic and cultural backgrounds. You'll need to be comfortable educating the caregivers on how to care for their loved one. This might include things like disease education, advance care planning, how to administer medications, and what to expect at end-of-life. You'll be provided with a company phone, and other equipment as applicable to perform your duties. You will coordinate care between your patient's community medical providers, and other disciplines to provide updates, obtain and place new orders. This coordination takes place during and in between visits. You will be part of an Interdisciplinary Team (IDT) consisting of a physician, nurse practitioner, registered nurse, and a medical social worker. The team meets each morning at 8:30 AM to review patient assignments and discuss complex cases as needed. There is also a monthly business meeting with our CMO. You'll also collaborate with inpatient hospice clinical liaisons to coordinate and facilitate patient care at end-of-life.
What we're looking for:
Education: Graduate of an accredited school of nursing, either through an AD, Diploma or BSN program. Graduate of a Masters degree in nursing program with a major or post-masters as a nurse practitioner. Certification in hospice/palliative care preferred (CHPN, ACHPN)
Licensure / Certifications: Current license to practice professional nursing in the State of North Carolina / South Carolina. Approval for practice as a nurse practitioner by the North Carolina Board of Nursing and the North Carolina Board of Medical Examiners.
Experience: Minimum of five years general medical experience in inpatient, outpatient and nursing home settings. Two years as a nurse practitioner preferred. Experience in an acute care setting desirable. Demonstrated competence in leadership and teaching. Demonstrated achievement and familiarity with administrative nursing techniques.
