Learning Services, A Collage Rehabilitation Partner is looking for a full-time Rehabilitation Case Manager in Gilroy, CA. Learning Services is a growing organization dedicated to excellence, honesty and ethics in the treatment of our clients and each other. We provide treatment for post-concussion syndrome, stroke, memory loss, neurobehavioral issues and accident or sports-related head injuries.

Responsibilities

The Case Manager is responsible for:
  • Ensuring the implementation of the client's treatment plan(s), from admission through discharge and follow-up
  • Coordinates schedules, therapies and other appointments/services
  • Responsible for regular contact with families and funders
  • Coordination with medical director and external medical providers to ensure appropriate medical care
  • Collaborates with the program nurse and clinical director to ensure clinical and medical needs are being met.
  • Communicates with the Operations Director to ensure residential care is being met and any personal preferences are identified.
  • Report writing, clinical documentation and facilitating funding issues as needed
  • Acts as a client advocate


Pay is based off of education and experience: $80,000 - $90,000 annually

A bachelor's degree along with case management in the healthcare industry/rehabilitation experience is
required. A master's degree and CRC or CCM designation is preferred
Must have a valid driver's license and maintain responsible driving behaviors
Excellent planning, organizational, multitasking, writing and time management skills are required
The ability to communicate with families, professionals and team members is a must
Strong clinical skills as shown by the ability to manage crisis situations, assess client's needs and help
formulate, prioritize and monitor treatment plans is necessary
The ability to demonstrate positive client interaction skills is essential
