What will you do in this role:

  • Coordination/case management of ancillary contributions including Physicians, Hospice, Dietician (not dietary), Consultations and Therapy.
  • Highly acute oversite to keep department operating within all federal, state, and local Skilled Nursing Regulations
  • Make frequent house rounds on all shifts to assess performance of nursing staff and to monitor the quality of care provided
  • In compliance with the Company's policies, is responsible for interviewing, hiring, developing, supervising and following the Company's performance management policy with assigned staff


  • Participate in internal audits of nursing practice
  • Establish and ensure that staffing levels and scheduling patterns are adequate and have appropriate coverage at Community
  • Assure development, maintenance, implementation and evaluation of the safety and infection control programs
  • Investigate and document incidents, identifies safety hazards and institutes corrective actions in a timely manner
  • Investigate and ensure completion of internal and state required incident reports for skilled nursing & assisted living communities including completion of appropriate interventions in a timely manner


  • Provide clinical expertise to facilitate and support the nursing process in resident interventions and care provided
  • Support Executive Director in planning and scheduling of staff training programs for Skilled Nursing, Assisted Living and Memory Care staff
  • Assist in pre-admission assessments of potential residents. Assists in orienting new residents to the physical environment and community culture
  • Responsible for making sure that all required electronic medical records are kept up to date
  • Ability to grow the skillsets of individual and collective team abilities


  • Communicate with residents and family members as needed to provide exceptional care and service


You have:

  • Valid and current LPN, RN, BSN state license required
  • Three years experience as a supervisor in a health care facility; Senior Living preferred.
  • Highly developed nursing skills; thorough knowledge in understanding and interpreting nursing philosophies, programs, and policies
  • Demonstrated commitment to high professional and ethical standards
  • Must have computer processing and electronic medical database experience such as LTC or Point Click Care or ALIS
  • Must be able to read, write and speak the English language
  • Ability to think critically


You might also have:

  • Three to five years of experience in Senior Care, Assisted Living and Memory Care setting
  • Demonstrated talent for interacting with a wide variety of people, particularly the elderly and their family members
  • Recognize current evolution of this role to include an 'Operations' emphasis


PHYSICAL REQUIREMENTS/WORKING CONDITIONS:

  • Ability to lift a minimum of 55 lbs. unassisted
  • Ability to push/pull 60 lbs. of resistance
  • Ability to stand and/or walk for duration of shift
  • Must be available to work weekends and holidays as needed
    May be exposed to unpleasant odors, contagious diseases and blood borne pathogens

Actual monetary compensation will be presented at the time a job offer is made and may not be within the posted range due to skill level, experience, industry specific knowledge, education/certifications and/or geographic location.
