Job Summary:
Under the supervision of the Chief of Clinical Services, responsibilities include the supervision of all licensed nursing staff. Ensures the proper assessment for and delivery of all nursing care. Provides focus and leadership to the nursing. Responsible for medication safety, triage, and clinical nursing across all PACE-RI sites as well as community-based nurses.

Physical Requirements:
Physical ability to regularly stoop, bend, kneel, lift, stand, walk, stretch, and reach for extended periods of time.
Must be able to regularly lift objects up to 25 pounds and at times up to 35 pounds.
Must be able to assist a 200-250 lb participant, using proper body mechanics.
Must have hearing and vision abilities within normal range (corrected) to drive safely and/or to observe and communicate with participants.
Must be able to navigate various departments of the organization's physical premises as well as related community care settings.
Must be able to tolerate conditions typically associated within a medical office and/or home care setting including potential exposure to bloodborne pathogens and infectious diseases.
Must be able to complete assignments in variable weather conditions and ever-changing and sometimes allergenic environments in participant homes, including extreme heat, humidity, dust, smoke, pet contamination, and cleaning supplies.
Experience:
5 Years of Nursing Experience, Required
3 Years of Experience in a Medical or Geriatric Setting, Preferred
2 to 5 Years Staff Management Experience, Required
1 to 3 Years of Program Management Experience, Preferred
License & Certification Requirements:
Rhode Island Registered Nurse License (RN)
Position Requirements:
CPR Certification
Driver's License & access to reliable transportation: community-based travel required.

Job Duties/Responsibilities:
- Provides leadership and clinical guidance to all licensed nurses in PACE day centers and in the home.
- Oversees the development of nursing plans of care for new and enrolled participants and collaborates with homecare team to ensure quality of care, completeness, and timely completion of all required assessments.
- Conducts qualitative and quantitative medical record audits to ensure proper delivery and allocation of services
- Engages in problem solving and critical thinking on a management level. Assists nursing staff to solve and manage challenging cases.
- Oversees the RN assessment and allocation of appropriate medication administration systems in the home and in the day centers.
- Serves on the Clinical Management Team to ensure quality clinical services and care plan integrity for participants of PACE.
- Serves as a role model for students and staff members.
- Oversees and coordinates orientation and competencies for all new licensed nursing staff.
- Completes yearly performance reviews and completes/oversees competencies for all licensed nursing staff.
- Protects privacy and maintains confidentiality of sensitive participant, employee, and agency information.
- Required to provide administrative on-call coverage on a rotating basis in order to assist in the delivery of 24-hour care to enrolled participants.
- Works with IDT to develop and implement protocols to respond to chronic illness and prevent health crisis/hospitalization.
- Is knowledgeable and adheres to all regulatory statutes regarding nursing standards of care, state, and federal regulations regarding PACE centers.
- Provides communication and problem solving to vendors with clinical/nursing/homecare issues.
- Oversees scheduling for all licensed nursing staff to maintain compliance with required participant needs.
- Participates in Quality Improvement Projects.
- Is responsible for supervision of all reporting staff members, performance evaluations, disciplinary actions, representation in management meetings and supervisory meetings.
- Assists in the selection and orientation of employees. Maintains appropriate level of training and orientation of staff members and provides on-going training as needed in department as identified.
- Assists with oversight, implementation and adherence to policies, procedures, and standards for all services in consultation with other management staff.
- Serves on various committees and meetings as requested. Participates in and contributes to the PACE management team.
- Communicates with participants, families, contracted providers, and the public regarding issues related to the routine operation of wellness services, resolves all grievances and complaints received related to department.
- Management and oversight of yearly flu and COVID-19 clinics. Works with other clinical managers to ensure a high level of compliance
- Collaborates with the Home Care Dept to assess for appropriate services in the home and makes recommendations and offers solutions to modify systems to best meet participants' needs.
- Works with the IDT and Chief of Clinical Services to control and prioritize utilization of home care services to reflect actual participant needs in conjunction with the Home Care Dept Manager.
- Create and implements departmental audit system to identify areas of non-compliance and opportunities for quality improvement; use data to inform decision making and action steps.
- Demonstrates an understanding of the importance of clear, thorough documentation as crucial to good participant care; completes and enters accurate, thorough participant documentation in the electronic health record; enters documentation for assessments, orders, and notes within required timeframes.
