JOB DUTIES
Demonstrates ability to adapt to varied age specific patient populations to include adult and geriatrics.
Performs assessment of physical, mental, and cognitive disabilities and disposition needs. Documents assessments
and reassessment utilizing the FIM's criteria.
Identifies teaching needs and goals related to the diagnosis of each patient from the admission assessment and by
collaborating with patient/family and the interdisciplinary team.
Participates wherever possible in initial evaluation team conference and all reevaluation team conference.
Communicates all pertinent information regarding changes in patients' status to appropriate staff, ex. (physician,
nurse, adviser, etc.)
Work closely with interdisciplinary team to ensure patients goals are met in the project time frames.
Initiates discharge planning on all patient from the day of admission anticipating family and environmental issues
which may effect discharge as well as patients needs. Works closely with case manager regarding discharge planning
and equipment needs.
Is knowledgeable of specific transfer techniques, use of equipment (teaching process, aware of need); ambulation
skills used with patients, all forms of ADL specific to patient's disabilities.
EDUCATION/EXPERIENCE
Valid Physical Therapist Licenses
