In this role you will:
- Work with nursing and primary care leaders foster the integration of BH and primary care.
- Work with team to ensure smooth operational workflow.
- Participate in program development.
- Participate in quality improvement activities.
- Focus on participating in and developing programming focused on health behavior change in the context of chronic disease management.
The successful applicant will offer:
QUALIFICATIONS:
- Masters or Doctoral degree in psychology, social work, or mental health counseling
- Independent clinical license to practice in the Commonwealth of Massachusetts (LISCW, LMHC, LMFT, PsyD, PhD)
- Previous training and experience working in an integrated primary care behavioral health team
- Bilingual English/Spanish a plus
