DEPARTMENT DESCRIPTION:
The Population Health Program must work closely with AACS clinics, across the HMC care continuum and across UW Medicine system to implement the evolving vision, goals and processes of population health in a rapidly changing environment. The AACS Population Health Program is a division level program with operations occurring across the care continuum and into the community, with close linkages with colleagues and leaders across UW Medicine. To manage operations and deliver a consistent and high-quality service to patients this position must partner and communicate frequently with clinical and operational leaders from HMC departments, AACS clinics (especially primary care), UW Medicine Population heath, care Management and others. This position represents the program operations in various venues. As program develops, may supervise additional staff.

PRIMARY JOB RESPONSIBILITIES:
Provide leadership and direction for population health program and staff including population health RN care managers across Harborview ambulatory care
Support and facilitate program development, implementation, evaluation, continual process improvement, and operational integration of population health processes and projects
Accountable for integration of value-based processes into primary care and specialty care, development of policy, procedures and standard work, and engagement in collaborative partnerships as part of population health
Manage the assigned budget and staff
Responsible to meet or exceed the program's strategic goals in clinical quality, service quality and financial outcomes established for the department
Maintain a work and care environment that is consistent with the ethics, mission, values and goals of Harborview medical center
Manage the overall safety of the clinic, ensuring adherence to clinical and research compliance policies and procedures
Hire, supervise, and maintain an effective team of employees, developing and assessing skill, ensuring training as needed
REQUIREMENTS:
At least 4 years of clinical experience with at least 2 years in ambulatory care, including care management activities and use of population health approaches
Demonstrated management/leadership and program management skills related to defined tasks, knowledge, skills, and abilities
Licensed to practice as an RN in WA State
Bachelor's degree in Nursing
