Summary:
The occupational therapist shall be responsible for providing a full range of occupational therapy including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position's qualifications, professional practices and ethical standards. The occupational therapist shall also demonstrate accountability for and contribute to program development, quality improvement, problem solving and productivity enhancement in a flexible interdisciplinary fashion.

CUSTOMER SERVICE:
Provide excellent customer service to all DHR customers. All employees are required to attend the DHR C.A.R.E.S program which outlines the Customer Service Principals including: Commitment, Accountability, Respect, Excellence and Service.
AGE SPECIFIC:
Employees must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served in his/her assigned unit. The individual must demonstrate knowledge of principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirement relative to his or her age.

POSITION EDUCATION/ QUALIFICATIONS:
- Graduate from an approved four year bachelor's degree program
- Current OT license & CPR
- Adult & Geriatric rehab experience is preferred
- CPI within 30 days of hire.
- Must be able to be sensitive to cultural and bilingual issues.
- Good written and verbal communication skills required.
- Ability to read, write and speak English
- Ability to communicate clearly and concisely with all levels of nursing, administration, and physicians
- Prefer hospital experience
Responsibilities:
- Promotes the facility mission, vision and values by effectively communicating them to others. Considers mission, vision and values in developing services, standards and practices
- Participate in pre-admission screening as requested and according to policy and procedure.
- Conduct initial assessment. Choose appropriate assessment tools, adaptive equipment and material/props used in the assessment based on the age and developmental level of the patient.
- Patient assessment is completed and in the medical record as outlined in policy & procedure. Data is recorded in the appropriate areas.
- Develop individualized treatment plans. Short and long term goals are written objectively and are measurable.
- Time frame for goal achievement is documented.
- Interventions are selected/documented based on assessment results and are appropriate for age, cognitive abilities and/or development level of the patient.
- Interventions are selected/documented and applied to support the interdisciplinary rehabilitation plan as evidenced by goal achievement.
- Consults with physician regarding referrals for other services as needed.
- Uses positive feedback to promote patient participation in their rehabilitation program.
- Patient response to the treatment plan is evaluated as evidenced by patient achievement of goals and/or goal revision, the treatment plan is updated as needed.
- Information is provided to patient/family regarding current status, program planning and discharge plans throughout the rehabilitation program.
- Weekly progress notes and discharge summaries are completed according to policy & procedure.
- Integrates family members into the treatment, educational and discharge planning process for a safe transition home, and to ensure continued care needs are met.
- Participates in clinical staffing and provides/seeks input to/from others on patient care.
- Function as a resource for and provide team members with inservice/cross-training on occupational therapy knowledge and skill areas.
- Participate in quality management activities. Attend meetings and uses reports/data to improve quality of care delivered. Submits data as requested.
- Participate in continuing education/professional development activities. Attends external professional growth activities as scheduled and communicates information to the team.
- Assist with the development of student programs as requested. Students are provided education coordinator/instruction as needed.
- Attends and participates in community education/promotional activities as requested.
- Maintain competency, licenses/ certifications, and memberships in applicable professional organizations.
- The competency/skills assessment is satisfactorily completed annually
Other information:
JOB KNOWLEDGE/EXPERIENCE:
- Three to five (3-5) years in-patient rehab experience is strongly preferred.
- Requires strong communication and organizational skills.
- Requires reasoning ability and good independent judgment.
- Must present a professional image.
- Requires working with frequent interruptions.
